Job Summary

The RD-Home Infusion Clinical Liaison serves as the nutrition liaison between referral sources, the home infusion pharmacy, and the interdisciplinary care team to support timely initiation and ongoing management of enteral nutrition therapy. Provides clinical nutrition assessment, coordination of services, and patient and caregiver education related to enteral feeding regimens, supplies, and equipment. Supports business development by building strong relationships with referral partners and promoting appropriate use of home enteral nutrition services. Collaborates closely with intake, pharmacy, nursing, and operational teams to ensure patients receive the right nutrition therapy at the right time.

Essential Functions

  • Conducts comprehensive nutrition assessments for patients referred for home enteral nutrition therapy to determine appropriateness of therapy and recommended nutrition plan.

  • Reviews and evaluates enteral nutrition referrals for clinical completeness, medical necessity, and alignment with physician orders, payer requirements, and organizational guidelines.

  • Collaborates with physicians, intake coordinators, pharmacists, and nursing staff to coordinate initiation and ongoing management of enteral nutrition therapy.

  • Develops individualized enteral feeding regimens, including formula selection, administration schedules, and tolerance considerations, in accordance with clinical standards and provider orders.

  • Provides education and counselling to patients and caregivers on enteral feeding therapy, including formula administration, pump use, tube care, troubleshooting, and nutrition goals.

  • Serves as a clinical nutrition resource for referral sources, supporting appropriate utilization of home enteral nutrition services and facilitating smooth transitions of care.

  • Supports business development efforts by building and maintaining relationships with hospitals, clinics, case managers, and providers to promote home enteral nutrition services.

  • Monitors patient progress, tolerance, and outcomes related to enteral nutrition therapy, communicating recommendations or concerns to the interdisciplinary care team.

  • Ensures documentation of nutrition assessments, care plans, and patient education is accurate, timely, and compliant with regulatory, accreditation, and payer standards.

  • Participates in quality improvement and performance initiatives related to enteral nutrition services, contributing data and clinical insight to support program growth and optimization.

  • Performs other duties as assigned.

Supervisory/Management Responsibilities

This is a non-management job that will report to a supervisor, manager, director or executive.

Minimum Requirements

  • Education - Credentialed as a Registered Dietitian by the Commission on Dietetic Registration (CDR)

  • Experience - No previous experience required

In Lieu Of

If hired on or prior to December 31, 2023, must have a minimum of a Bachelor's degree in nutrition or dietetics AND Credentialed as a Registered Dietitian by the Commission on Dietetic Registration (CDR)

Required Certifications, Registrations, Licenses

  • Licensed as Registered Dietitian in the state the team member is working.

  • Registered Dietitian licensure from the CDR

Knowledge, Skills and Abilities

  • Maintain 75 hours of continuing education every 5 years

  • Ability to interpret data, teach, communicate to other healthcare professionals, manipulate numbers and write in medical record
