Position Summary

We are seeking a skilled and compassionate Licensed Vocational Nurse (LVN) with hands-on experience in gastrostomy tube (G-tube) medication administration and enteral feeding. The LVN will provide direct nursing care and clinical oversight to individuals with complex medical needs in a residential or home care setting, ensuring safety, compliance, and high-quality care.
Key Responsibilities

  • Administer medications via gastrostomy tube (G-tube) following physicians orders and established protocols
  • Perform and monitor enteral feeding (bolus and/or continuous feeding)
  • Assess G-tube site for signs of infection, leakage, or complications
  • Maintain accurate Medication Administration Records (MAR) and treatment documentation
  • Monitor vital signs and report changes in condition to the physician and care team
  • Provide tube care, flushing, and troubleshooting (e.g., clogging prevention)
  • Educate and train direct care staff (DSPs/caregivers) on proper G-tube support procedures
  • Ensure compliance with Title 22 regulations, infection control standards, and agency policies
  • Respond to medical emergencies and implement appropriate interventions
  • Collaborate with physicians, pharmacists, and interdisciplinary team members
Qualifications

  • Active Licensed Vocational Nurse (LVN) license in the State of California
  • Minimum 12 years of experience with G-tube medication administration and feeding
  • Current CPR/First Aid certification
  • Strong knowledge of infection control and medication safety
  • Experience in RCFE, ARF, or home care settings preferred
  • Ability to work independently and as part of a team
  • Excellent documentation and communication skills
