The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.

Responsibilities:

To provide timely, comprehensive, evidence-based nutritional care and education to patients and families. To prepare patients and families as they transition to a new enteral nutrition requirement mainly through patient education on home enteral nutrition modalities. Coordinates with inpatient and outpatient teams and pharmacy technicians to ensure patient success. Demonstrates advanced knowledge in area of expertise related to high-risk populations. Participate in the training of medical staff, dietetic interns and peers. Ensure best practices by regularly reviewing the literature and changing policies/practice, if warranted. Available for travel to hospitals, home visits and clinics for patient assessment and education.

ESSENTIAL FUNCTIONS

  • Home Nutrition Support Education - Provides education on medical nutrition therapy and food-drug interactions to patients and caregivers. Ensure continuity of care when a patient transitions home. Provides patient/caregiver education on gravity, syringe or pump based enteral nutrition feedings as well as supply care. Ability to identify different enteral access devices and their appropriateness in different clinical settings. Instruction on general tube site care. Instruction on flushes and general medication administration tips. Education to providers on script writing and insurance nuances. Develop nutrition education materials.
  • Communication - Effectively communicates nutrition plans and rationale to team members. Use hospital wide and pharmacy specific electronic medical records. Liaison between pharmacy team and inpatient team. Assists providers with order writing. Demonstrates ability to utilize various HIPAA secure patient communication modalities. Possesses the technological knowledge to utilize software programs pertinent to role. Maintains communication with industry representatives to remain aware of supply chain issues, technology, and formula changes.
  • Interdisciplinary Teamwork - Educating inpatient and outpatient provider teams, care coordinators, dietitians and other healthcare professionals on home enteral nutrition and insurance nuances. In-services/lecture requests. Mentors clinical nutrition specialists and clinical dietitians. Serves as clinical preceptor to dietetic interns and provides didactic training. Participates in nutrition support rounds, offers insights and teaching points when appropriate.
  • Insurance - Understanding of insurance coverage criteria for home EN and documentation required to support claims. Uploading documentation to pharmacy EMR as needed. Awareness of insurance and industry changes.
  • Clinical Nutrition Assessment - Assesses and presents nutrition status of patients to the medical team. Provides ongoing nutritional assessment, monitoring, evaluation, and education to patients and families. Develops and implements individualized nutrition care plans for patients across the lifecycle, from infants to the elderly, as assigned. Integrates evidence-based research in the education, practice and management of patients. Verifies clinical appropriateness of proposed home nutrition support plans. Maintains level of expertise in formula choice & modality to optimize success with home EN. Expert in lab monitoring for long term EN; optimizing compliance with home EN and troubleshooting.
  • Quality Assurance/Research - Leadership role in a QA activity, hospital committee or nutrition related research in coordination with other services. Analyze data and participate in QA activities to identify opportunities for improved practice. Attend regular department staff meetings as scheduled. When opportunities arise, participates in nutrition and metabolic research conducted independently or in coordination with other services/providers; prepares and submits materials for publication. Authors a review article or book chapter. Speaks at local, regional or national conference.
  • Travel - Home visits for nutrition assessments and education. Traveling to different hospitals, doctor's offices, different team meeting spaces
  • Other duties as assigned


MINIMUM EDUCATION & EXPERIENCE

  • Bachelor of Science degree in Nutrition, Foods and Nutrition Service or equivalent, completion of one-year dietetic internship experience or equivalent required.
  • Master's Degree in related field preferred.
  • 4 years of clinical nutrition experience in a health care setting required.


KNOWLEDGE, SKILLS AND ABILITIES (Required)

  • Demonstrate proficient and competent performance of required tasks of Clinical Nutrition Specialist I.


LICENSES AND CERTIFICATIONS (Required)

  • Registered by the Commission on Dietetic Registration, the credentialing agency of The Academy of Nutrition and Dietetics.
  • Advanced certification, CNSC, CDCES, CSP or other specialist certification in specific area of practice required within 18 months of hire.

MISC. (Required)

  • A reliable personal vehicle and a NYS driver's license.
