As an LCSW with Gotham Enterprises Ltd, you will play a critical role in delivering psychosocial support to patients and their families. Your daily responsibilities will include:

• Conducting comprehensive clinical assessments to evaluate patient needs and develop personalized treatment plans.
• Implementing evidence-based therapeutic interventions such as cognitive-behavioral therapy (CBT) and solution-focused therapy, tailored to the unique circumstances of each patient.
• Maintaining accurate, timely clinical documentation in compliance with HIPAA regulations, ensuring patient confidentiality and care continuity.
• Collaborating with interdisciplinary team members, including physicians and nurses, to enhance the patient's overall care experience.
• Facilitating family meetings to discuss care options, addressing emotional concerns while providing education on available resources.
• Participating in case consultations and peer review meetings to assess care quality continually and integrate new methodologies into practice.
• Staying abreast of the latest research and best practices in social work and mental health fields to ensure a high standard of care.

You will work primarily with hospice patients and their families, who are facing significant emotional stress and complex psychosocial issues related to terminal illnesses. Your expertise will be vital in helping them navigate this challenging time, offering emotional support, coping mechanisms, and education on the grieving process and available community resources.

• A Master’s degree in Social Work from an accredited institution is required.
• A valid LCSW license in the state of Illinois.
• A minimum of three years of clinical experience, preferably in hospice, palliative care, or a related field.
• Proficient in various psychotherapy modalities and the application of evidence-based practices.
• Exceptional communication skills, both verbal and written, with the ability to foster strong therapeutic relationships.
• A thorough understanding of ethical guidelines and standards established by the National Association of Social Workers (NASW).

• Experience in hospice or palliative care settings, particularly working with diverse patient populations.
• Familiarity with community resources and support services available for patients and their families.
• Proficiency in using electronic health records (EHR) systems for documentation and care coordination.
