Responsibilities:
JOB SUMMARY
The Care Coordinator II collaborates with the primary physicians, research team, and other healthcare team members in the development of patient centered goals and actions, that represents the total care needs and resources of the patient/client and family. The Care Coordinator II acts as patient advocate through the continuum and is available to the physicians, the research team, patient and family as a resource to facilitate communication. Responsible for providing support and care coordination to assigned patients to ensure understanding of care plan, and effective transitions as patients move through the healthcare and clinical research continuum. Serves as an embedded care resource with focus on the health and wellness and care coordination needs of the patient population. Works closely with clinical and research staff to collect, store, and maintain data in accordance with FDA regulations, International Council of Harmonization, and Good Clinical Practices.
ESSENTIAL FUNCTIONS
  • Assists in the coordination and management of patient care needs in the clinical and the clinical research environment. Maintains and manages their caseloads working independently with minimum direction, anticipate and organize work flow, work with a high volume caseload, and deal with rapidly changing priorities. Works collaboratively with clinical and research staff, social service organizations and community resources, as needed, to meet each patient's needs. Maintains a working knowledge of and can effectively communicate patient associated costs of research and non-research related costs, applicable laws and regulations, government (Medicare, Medicaid, SSI, and SSDI), and insurance benefits. Performs other duties as assigned. Ensures accurate and complete documentation is recorded for every patient encounter. Utilizes active listening, health literacy principles, including teach back to ensure effective communication with patients and caregivers.
  • In conjunction with clinical and research practitioners such as RNs, doctors, physician assistants, and clinical research staff, coordinates and reviews patients' progress and monitors and reports patients ability to manage healthcare issues. Performs inbound and outbound calling, medical records requests, appointment scheduling, physician/NP orders, and completing referrals requests as necessary based on patient need. Communicates professionally and using all modalities (email, technology, written, and verbal) with clinical and research team, patients, and care givers in a way that is clear and concise.
  • Assists leadership with monthly quality check of recorded and submitted data including gathering and reporting metrics and compliance with other deadlines. Participates in precepting and training of staff on care coordination procedures, tasks, and systems. Assist with team guideline adherence to timely preparation of patient documents and request reconciliation. Participates in efforts to improve processes and procedures could include participating in taskforces, committees, and special projects.
  • Ensures safe handoff of care transitions into clinical care and/or in clinical trials from Care Coordinators to other healthcare or research staff. Coordinates follow-up care with PCP/ Specialists regarding outpatient follow-up appointment and plan of care. Coordinates care with internal and external providers and healthcare team members involved in the care. Builds and maintains working relationships with patients and their care givers and other health care partners. May represent Care Coordination team in a variety of meetings with internal and external representatives.
  • Assists in the creation and implementation of tools to continuously monitor team performance. Maintains working knowledge of current clinical research standards. May participate in coordination of clinical research studies as needed.
  • Performs other duties as assigned.

EDUCATION
  • Bachelors in health science or 4 years health science-related experience Required

EXPERIENCE
  • 3 years Direct patient care, outpatient, ambulatory care, specialty care, or community healthcare experience Required
  • Other Strong writing and computer skills, including MS Word, Excel, EMR (electronic medical record), medical terminology. Preferred

LICENSE AND CERTIFICATIONS
  • Basic Life Support (BLS) - Certification CPR Card Required
  • Certified Medical Assistant (CMA) - Certification Medical Assistant Certificate Preferred
