What You'll Do:
  • Completes an initial, comprehensive and ongoing assessments of patient and family to determine home health or hospice needs.
  • Provide respectful, compassionate care in patients' homes.
  • Develop patient focused care plans that establish goals based on nursing diagnoses, and incorporate rehabilitative nursing actions.
  • Collaborate with interdisciplinary team members.

What You Bring:
  • Active nursing certification (state-licensed).
  • Valid driver's license, reliable transportation, and current auto insurance.
  • Strong communication skills and a team-oriented mindset
