Job description

DESIGNATED MANAGER/COORDINATOR (DM/DC) is to provide program management and high level oversight of services to ensure State licensing standards/compliance throughout the program. Staff compliance, and program quality. Overseeing and evaluating service effectiveness for people with disabilities. DM/DC will conduct client intake, admission, assessments and service plan development for clients. Attend client Support Team meetings as needed. Develop and implement client CSSP/A’s. Review client files to ensure that they meet licensing guidelines by keeping a working knowledge of regulatory requirements. Maintain accurate records and reports. Serve as a communication liaison with guardians and other service providers. Review incident and emergency reports, and implementation of correctives actions.

QUALIFICATIONS:

Candidate must meet the Designated Manager qualifications in accordance with the MN Statutes 245D requirements through one of the following:

  • 4-year degree in a human service field and a minimum of 1-year direct care experience
  • 2-year degree in the human service field and a minimum of 2 years of direct care experience
  • Diploma/certificate from Post-Secondary School in a human service field and a minimum of 3 years of direct care experience
  • Minimum of 4 years of direct care experience under the supervision of a Designated Manager
  • Minimum of 3 years of progressively responsible supervisory and personnel management experience in Human Services.
