What You'll Be Doing:
The Eligibility Coordinator is responsible for gathering the required documentation for qualifying residents to move into the community, including working with the resident and family to complete and obtain the necessary documentation, communicate with all necessary parties, coordinate assessment appointments and prepare the resident’s administrative file for submissions for approval. The Eligibility Coordinator has a unique opportunity to make an immediate impact and inspire purposeful lives for all.

What we're looking for:
  • High School Diploma or equivalent
  • Two (2) or more years of experience working with state services agencies such as Medicaid, HUD or LITC
  • Strong customer service orientation
  • Ability to manage multiple tasks successfully.
  • Bachelor’s Degree in Social Work, preferred.
  • Valid Driver's License with good driving record and proof of automobile insurance
