Job description
Provides oversight for planning, implementation and evaluation of the DSME/T program and ensures the systematic and coordinated delivery of diabetes educational services. Reports to the Wellness Manager.
Responsibilities
1. Diabetes Education Program Development/Oversight
a. Plans, coordinates, implements and evaluates the activities and curriculum of the Diabetes Education Program in a manner consistent with practice standards and efficiently maintains appropriate documentation of education in electronic medical record and statistics in order to maintain national program accreditation.
b. Develops and maintains relationships and partnerships with community groups, payers and potential referral sources. Interfaces with the Volunteer Diabetes Program Accreditation Advisory Group.
c. Efficiently performs assessments of referred patients in a caring manner, educates and evaluates participants and serves as a resource to patients upon program completion.
d. Oversees the diabetes education process to ensure that services are provided in an individualized and fiscally feasible manner.
2. Quality
a. Develops and directs the implementation of an annual program evaluation plan and performance improvement activities.
b. Performs high quality assessments of infant, pediatric, adolescent, adult and geriatric populations and maintains awareness of the age and literacy level of the patient served when documenting, planning care and providing follow up services.
c. Observes and adheres to quality standards. Perform systematic annual review of the program in order to foster the provision of quality health care services by the program and practice and ensures that all services are provided with compassion, understanding and respect for others.
d. Plans and coordinates high quality continuing education programs in order to ensure that practice personnel receive high quality clinical training and education. Act as a clinical consultant to practice staff in a manner that promotes the provision of high quality care by the practice staff.
e. Promotes the quality and efficiency of his/her own performance by remaining current with the latest trends in field of expertise through participation in job-relevant seminars and workshops, attendance at professional conferences and affiliations with national and state professional organizations.
3. Collaboration & Partnership
a. Develops positive relationships between the center and community agencies, special interest and civic groups and professional affiliations by acting as the center's representative. Works through civic groups and public education activities to improve community health and foster the centers image as a leader in the health care industry.
4. Other Duties as Assigned
a. Coordinates with Business Development in planning and executing all marketing activities.
b. Performs other duties as assigned or requested.
Qualifications
DescriptionMinimum RequiredExperience5 years clinical experience as a Registered Nurse or Registered Dietician, with 2 years in diabetes education and Certification in Diabetes Education; Education in, and/or experience with, chronic diseases and disease self-management.EducationDegree from accredited school.TrainingKnowledge about chronic disease management and disease self-management educational processesSpecial SkillsKnowledge about program management; Good oral & written Communication, interpersonal, critical thinking, organizational, time management & computer skills. Proficient in various computer applications, including spreadsheetsLicensureRegistered Nurse or Registered Dietician; and CDE certification by national Certification Board of Diabetes Educators; BLS Certification
