• Initiate discharge planning from the day of admission, meticulously assessing patient needs to ensure a smooth transition post-treatment.
• Conduct comprehensive discharge sessions with patients, ensuring they have a clear understanding of coping skills, transition plans, and resources available to them.
• Verify and confirm discharge appointments with patients and their loved ones to facilitate a well-coordinated exit process.
• Engage with referral sources and outpatient providers to maintain continuity of care throughout the treatment journey, from admission to discharge.
• Oversee and facilitate patient transfers to higher levels of care as required, maintaining documentation and communication with associated healthcare professionals.

The primary focus of this role is on individuals undergoing detoxification from substance use disorders. This population often presents unique challenges requiring a high level of empathy, patience, and a robust understanding of both mental health and addiction issues. You will work with diverse patients, tailoring treatment approaches to meet their specific needs while supporting them through their recovery journey.

• A Master’s degree in Social Work from an accredited institution.
• Active license as a Clinical Social Worker (LCSW) in the state of California.
• Proficient written and verbal communication skills for effective patient interaction and collaboration with healthcare team members.
• A proven ability to establish and maintain professional boundaries while providing emotional support to clients.
• A meticulous approach to documentation, ensuring that all records are completed and submitted in a timely manner.

• Prior experience working within a detox or addiction recovery setting is highly desirable.
• Familiarity with behavioral health care practices, treatment methodologies, and interdisciplinary collaboration.
