Experienced PDN Pediatric/Adult RN Case Manager

Immediate opening for an experienced PDN Pediatric/Adult RN Case Manager.

Job Description:

The PDN Case Manager is an integral part of the interdisciplinary care team and as such is responsible for admitting and coordinating appropriate care for each patient and family.

Main job duties include and are not limited to:

  • Oversees assigned patient cases to ensure the highest level of care to patients and families.
  • Functions as a case manager by completing comprehensive initial and ongoing assessments of patient and family to determine needs.
  • Develops and oversees plan of care with patient and family and manages team resources for a positive patient outcome.
  • Provide training to nurses for TRACH, VENT AND G-TUBE care
  • Provide a complete continuum of quality care through close communication with insurance case managers
  • Providing full Case Management services;
  • Assessing needs
  • Establishing a plan of care
  • Service coordination
  • Community resource utilization
  • Implementing the plan of care
  • Evaluating the plan of care for effectiveness
  • Serve as a resource to patients and their families/caregivers
  • Full management of prior authorization requests for PDN & RESPITE
  • Any other duty assigned by the Manager, Director of Clinical Services & Compliance Officer
