Description:

GENERAL SUMMARY

The Patient Navigator RN will assist in the assessment, planning, implementation, evaluation, and documentation of nursing care in accordance with facility, department, and professional nursing practice. The Navigator will be responsible for helping patients to understand their diagnosis, coordinate their treatment plans, and communicate with providers and staff. Using a combination of communication strategies, resources, and tools, the Navigator will assist patients to anticipate, identify, and achieve effective steps through the various treatment options available.

ESSENTIAL JOB FUNCTION/COMPETENCIES

Responsibilities include but are not limited to:

  • Guides patients and families through their healthcare experience, answers questions, provides education and helps them understand their diagnois and treatment options.
  • Follows patients from date of diagnosis through the completion of the care-path to ensure all treatment options are explored.
  • Reviews prior patient history and provides updated information in the medical record in an appropriate time frame.
  • Reviews available information obtained by other health care team members, internal and external.
  • Communicates problems with workflow, failure to understand directions or procedures, conflicts and other concerns encountered to the manager or director of clinical services.
  • Collaborates with surgical scheduling department regarding patient preparation, required clearances, review and reporting of abnormal pretesting results and complex issues relating to the procedure.
  • Maintains professional and technical knowledge by attending educational workshops, reviewing professional publications, establishing personal networks and particpating in professional societies.
  • Performs other position related duties as assigned.

CERTIFICATIONS, LICENSURES OR REGISTRY REQUIREMENTS

  • RN License and certification as required by state.
  • BLS certification required.

KNOWLEDGE | SKILLS | ABILITIES

  • Excellent verbal and written communication skills.
  • Excellent organizational skills and attention to detail.
  • Ability to build relationships with patients and display empathy and compassion to patients.
  • Knowledge in healthcare systems operations such as EMR.
  • Skill in using computer programs and applications including Microsoft Office.
  • Complies with HIPAA regulations for patient confidentiality.
Requirements:

EDUCATION REQUIREMENTS

Graduate of an accredited nursing program required. BSN preferred.

EXPERIENCE REQUIREMENTS

2 years' previous clinical experience.
