We are seeking compassionate, dependable, and energetic professionals to provide in-home and community-based support to individuals with developmental and intellectual disabilities. The ideal candidate will provide person-centered care and support while encouraging independence, community involvement, and meaningful life experiences.

What You'll Do


As a Community Support Professional, you may:

  • Play games and participate in recreational activities.
  • Watch movies and engage in leisure activities.
  • Assist with arts and crafts projects.
  • Support individuals in participating in community activities.
  • Encourage independence and social interaction.
  • Provide companionship and positive role modeling.

Community Activities May Include

  • Parks and recreation programs
  • Shopping trips
  • Zoo visits
  • Do Space activities
  • Library visits
  • Movies and entertainment events
  • Community events and outings based on individual interests

Schedule

This position offers flexible scheduling based on the needs of the individuals and families we support.

Current availability needed includes:

  • Late afternoons
  • Evenings
  • Weekends
  • Additional hours as assigned

Essential Job Responsibilities

  • Maintain a safe and supportive environment at all times.
  • Assist individuals in accessing and participating in community activities and services.
  • Provide transportation to approved activities, appointments, employment sites, and community events.
  • Implement Behavioral Support and Habilitation programs as authorized and directed by DHHS.
  • Utilize approved crisis intervention techniques when necessary while maintaining dignity, respect, and safety.
  • Support individuals in achieving personal goals and increasing independence.
  • Assist families with caregiving responsibilities as outlined in the individual's service plan.
  • Complete required documentation accurately and in a timely.
  • Follow all company policies, Medicaid requirements, and confidentiality standards.

Qualifications

Preferred Qualifications

  • Minimum one year of experience working with individuals with developmental disabilities, intellectual disabilities, or special needs.
  • Experience supporting individuals with behavioral challenges.
  • Strong communication and problem-solving skills.
  • Ability to adapt activities to meet individual needs and abilities.

Required Qualifications

  • Must be at least 18 years of age.
  • Valid driver's license and reliable transportation.
  • Ability to pass all required background checks.
  • Ability to transport individuals safely within the community.
  • Ability to maintain confidentiality in accordance with HIPAA requirements.

Physical Requirements

  • Ability to lift, transfer, and assist individuals as needed.
  • Ability to stand, walk, bend, and participate in community activities throughout the day.
  • Ability to safely operate a motor vehicle.
