LPN, Client Relationship Specialist

Schedule: Friday, Saturday, Sunday, Monday — 8:30am–5:00pm

When patients are ready to transition home, HCR is the name our referral sources rely on for smooth coordination, responsive communication, and exceptional clinical service. Every member of our team plays a vital part in making that happen.

As the Client Relationship Specialist, you become the driving force behind those transitions. You’ll build strong relationships with community partners, ensure referrals are processed quickly and accurately, and position HCR as the provider of choice for patients needing home care. Working Friday through Monday, 8:30am–5pm, you’ll support high-volume referral periods where your expertise has a direct and meaningful impact.

What You’ll Do

• You will build trusted relationships with physicians, hospitals, and rehab partners—helping them confidently choose HCR for their patients’ home care needs.

• You coordinate and process incoming referrals quickly, accurately, and professionally across multiple service lines throughout New York State.

• You will communicate proactively with referral sources and internal teams, ensuring a seamless, customer-focused experience from initial call to service initiation.

• You secure the resources patients need by arranging durable medical equipment, oxygen, and other critical services with outside vendors.

• You will verify insurance coverage and collaborate with Insurance Specialists to streamline authorizations and reduce delays.

• You document clearly, follow all regulatory requirements, and contribute solutions and insights during team and agency meetings.

• You maintain expert knowledge of HCR programs and services, positioning yourself as a reliable, go-to resource for referral partners.

• You perform other related duties as assigned.

• You have an LPN Nursing degree.

• You hold a current, valid NYS LPN license.

• You will demonstrate exceptional customer service, strong communication skills, and confidence interacting with external providers.

• You are detail-oriented, efficient, and comfortable working in a fast-moving, high-volume environment.

• You will meet or exceed productivity expectations and contribute to department goals with a solutions-driven mindset.

• You bring provider relations or referral experience within a healthcare setting.
