WHAT YOU CAN EXPECT
- Assess, monitor, and implement treatment plans for patients at various stages of the disease, focusing on prevention of complications and management of stable diabetes.
- Educate patients and families on various subjects, including diabetes and the disease process: medication, diet and nutritional monitoring; foot care, prevention and care of complications; blood glucose monitoring, and the techniques of self-monitoring and injection; technology, understanding new developments.
- Provide ongoing assessment and recommendations to the treatment team and patient/family for adjustments in the plan of care, as required.
- Develop and individualize interdisciplinary nutrition care plan with short and long-term goals.
- Create an educational setting conducive to learning with adequate resources to facilitate the learning process.
QUALIFICATIONS
- Required Associates or Bachelors Degree in Nursing or RN Diploma or Bachelors Degree in Dietetics/Nutrition
- Required Registered Nurse (RN) - State Licensing Board or Registered Dietitian (RD) - State Licensing Board
- Required Basic Life Support Program (BLS) - American Heart Association
- Preferred 1 year Nutrition/Diabetes Care Preferred
- Preferred Certified Diabetes Educator (CDE) - National Certification of Diabetic Educators
