The Diabetes Educator performs professional consultations, encourages healthy lifestyles, and helps to educate individuals, families, and communities about diabetes. In this role you will provide medical therapy services to patients, including assessment of nutritional status, nutrition care planning, counseling, and evaluation. You will also provide individualized and age-specific diabetes self-management education. This position acts as a liaison between the provider and the patient to provide quality education.

WHAT YOU CAN EXPECT
  • Assess, monitor, and implement treatment plans for patients at various stages of the disease, focusing on prevention of complications and management of stable diabetes.
  • Educate patients and families on various subjects, including diabetes and the disease process: medication, diet and nutritional monitoring; foot care, prevention and care of complications; blood glucose monitoring, and the techniques of self-monitoring and injection; technology, understanding new developments.
  • Provide ongoing assessment and recommendations to the treatment team and patient/family for adjustments in the plan of care, as required.
  • Develop and individualize interdisciplinary nutrition care plan with short and long-term goals.
  • Create an educational setting conducive to learning with adequate resources to facilitate the learning process.

QUALIFICATIONS
  • Required Associates or Bachelors Degree in Nursing or RN Diploma or Bachelors Degree in Dietetics/Nutrition
  • Required Registered Nurse (RN) - State Licensing Board or Registered Dietitian (RD) - State Licensing Board
  • Required Basic Life Support Program (BLS) - American Heart Association
  • Preferred 1 year Nutrition/Diabetes Care Preferred
  • Preferred Certified Diabetes Educator (CDE) - National Certification of Diabetic Educators
