In addition, specific functions may change from time to time:Function as a part of the leadership and coordination team (Medical Director, Physicians) of the Pediatric and Adult Diabetes ProgramCollaborate with Information Systems staff for the maintenance of the diabetes data collection systemCollaborate with diabetes team and outside providers in the development and implementation of individualized educational plan of care for each patientAssess health status, learning needs, and self-management process of referred patients with initial class presentation and with follow-up contacts per program guidelinesFacilitate development and use of protocols and plans patient care in cooperation with the diabetes center teamDiscuss plan of care with the patient and family at the direction and discretion of physiciansMonitor self-management processes based on assessment of physical, psychological, and spiritual responses and financial needs of patients, families, or significant others and alerts the health care team to actual and potential health related problems, and unusual occurrencesEvaluate care and coordinates interventions of referred patients in conjunction with all members of the healthcare teamDevelop strong supportive relationships with referred patients, hospital departmental personnel, physicians, other disciplines and community-based agencies and community physiciansMaintain knowledge of and participate in CQI and QAI activitiesConduct outcomes evaluations including effectiveness of follow-up procedures and make results available to physicians and the multidisciplinary teamRetrieve and report outcomes data indicators that show the impact of diabetes education on patient health status, and reports results to the healthcare team and the collaborative practice groupEnsure data is collected and reported to the ADA for continued certification of the diabetes programConducts periodic diabetes education programs to staff and community groups in coordination with Education Department as requiredResponsible for raising community awareness about diabetes education through public service programs in coordination with Marketing Division as appropriateGuide and educate assigned interns and student dietitians during their rotation with the Diabetes ProgramEnsure compliance with all local, state, and federal requirements for documentation and registration of outpatients per CFVHS corporate compliance and other policiesMaintain and adhere to policies and procedures that are consistent and in compliance with the American Diabetes Association’s nationally recognized program requirementsOther duties as assignedMinimum QualificationsThe following qualifications, or equivalents, are the minimum requirements necessary to perform the essential functions of this job:Education and Formal Training:Bachelor’s degree in Health Sciences or related healthcare field requiredCurrent healthcare licensure (in field of study) in North Carolina requiredCurrent Certified Diabetes Educator (CDE) requiredRegistered Dietitian preferredWork Experience:3 years of current hospital experience in clinical area of responsibility required1 year management experience preferred1 year case management experience preferredKnowledge, Skills, and Abilities Required:Maintain current knowledge and skills in diabetes self-management and education concepts, practices, and techniques and disseminates information to the health care team through appropriate forumsPositive interpersonal communications skillsOutstanding verbal and written skillsDemonstrate effective organization, prioritization, and leadership skillsKnowledgeable of the age specific physiological, psychosocial, developmental, and spiritual needs of the patient population typically servedPhysical Requirements:Ability to stand and walk for extended periodsMay be required to bend and liftAbility to access all areas of the hospital including detached buildingsExcellent speech, auditory and visual acuity for communication of patient needs and observation of patientsPosition may involve skin, eye, mucous membrane, or parenteral contact with blood or other potentially infectious materials#CFVH-2025Required Licenses and CertificationsCDE - National Certification Board For Diabetes EducatorsCape Fear Valley Health System is an Equal Opportunity Employer M/F/Disability/Veteran/Sexual Orientation/Gender Identity #J-18808-Ljbffr
