Summary
The Nursing Case Manager is responsible for overseeing the health and medical care of individuals supported by Bello Machre. This role involves coordinating medical services, developing health plans, and ensuring compliance with all healthcare regulations. The ideal candidate is compassionate, detail-oriented, and committed to enhancing the well-being of individuals with developmental disabilities.

Basic Qualifications:

To perform this job successfully, an employee must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skills, and/or ability required to successfully perform the essential functions of this job. In addition to the requirements below, the employee must be at least 18 years of age, have a nurturing and caring demeanor, and have a genuine interest in supporting the independence of those we serve.

Education and/or Experience
Registered Nurse (RN) license required; Bachelor of Science in Nursing (BSN) preferred. Minimum of 2 years of nursing experience, preferably in developmental disabilities or community health.

Trainings, Licenses, and Certifications CPR and First Aid certification required.

The employee must have and maintain a valid Maryland driver’s license. They may not have more than three points on their license at any time and may not have any alcohol and/or drug related citations on their license.

Clinical Care Coordination

  • Develop, implement, monitor, and update individualized nursing plans of care based on assessed healthcare needs.
  • Coordinate routine, specialty, and emergency medical services for individuals receiving support.
  • Monitor changes in health status and collaborate with healthcare providers to ensure appropriate follow-up and continuity of care.
  • Communicate effectively with physicians, specialists, pharmacies, families, guardians, and interdisciplinary team members regarding medical needs and recommendations.

Health Oversight & Regulatory Compliance

  • Conduct nursing assessments, health monitoring, and wellness reviews in accordance with organizational policies and regulatory requirements.
  • Ensure compliance with all applicable federal, state, and local regulations, including Maryland Board of Nursing delegation requirements and Developmental Disabilities Administration standards.
  • Maintain accurate, timely, and confidential medical documentation, including medication records, controlled substance documentation, medication destruction records, health assessments, nursing notes, and other required clinical documentation.
  • Monitor delegated nursing tasks and provide ongoing clinical oversight to promote safe, quality healthcare services.

Education, Training & Professional Support

  • Provide education, instruction, and ongoing coaching to direct support professionals and other team members on healthcare procedures, medication administration, infection prevention, emergency response, and other health-related topics.
  • Assess staff competency and reinforce best practices through observation, mentorship, and continuing education.
  • Support the development and delivery of organizational healthcare training programs and educational initiatives.
  • Participate in orientation and onboarding activities for new employees as assigned.

Advocacy & Interdisciplinary Collaboration

  • Advocate for the health, safety, rights, and well-being of individuals served.
  • Participate as an active member of interdisciplinary teams to promote person-centered care and positive health outcomes.
  • Collaborate with internal departments and external healthcare partners to ensure coordinated, high-quality services.
  • Assist with quality improvement initiatives, audits, investigations, and other departmental projects as assigned.

On-Call & Clinical Support

  • Participate in the nursing on-call rotation, including evenings, weekends, and holidays, as scheduled.
  • Provide clinical guidance and support during health-related concerns and emergencies.
  • Respond appropriately to urgent healthcare situations and assist with follow-up as needed.
