Job Description

We are a Maryland DDA provider agency seeking a residential Registered Nurse Case Manager/Delegating Nurse to join our team! This is a field position primarily working from a home office with some minimal time spent at our corporate offices. You will be responsible for overseeing the health care needs of an assigned caseload of persons with intellectual/developmental disabilities.

Responsibilities:

· Complete initial nursing assessment of new customers

· Conduct on-site supervisory visits and assessments of customers on your caseload minimally every 45 days

· Provide training and ongoing supervision of unlicensed personnel in the administration of medication, treatments and nursing care

· Establish, update and oversee implementation of the nursing plan of care

· Provide case management, serving as a liaison between the customer and their health care providers

· Provide on-call telephone triage support on a rotational basis

· Attend interdisciplinary meetings as needed

· Attend required Maryland DDA trainings

· No weekends or holidays involved, with the exception of the rotational on call telephone triage

Qualifications:

· Experience in developmental disability nursing/delegating nursing preferred

· Strong critical thinking and assessment skills

· Comfortable and adept at using technology and eMAR systems

· Excellent communication and interpersonal skills

· Training/supervision experience preferred

· Ability to thrive in a fast-paced environment
