The pay range for this position is $50,000 - $65,000 annually, set in good faith based on the responsibilities of the role, qualifications, and market conditions.
Job Summary
The Case Manager plays a critical role in supporting client continuity of care by coordinating discharge planning, facilitating Medicaid enrollment, scheduling follow-up appointments, and ensuring seamless transitions between levels of care. This position collaborates closely with clinical, medical, admissions, and external providers to support client stability, compliance, and long-term recovery outcomes.
Key Responsibilities
  • Coordinate comprehensive discharge planning for clients, beginning at admission and continuing throughout treatment
  • Facilitate Medicaid enrollment, recertification, and eligibility verification for clients as appropriate
  • Schedule, coordinate, and confirm appointments during active treatment and for aftercare, including primary care, behavioral health, psychiatry, MAT providers, and community supports etc.
  • Arrange and coordinate client transportation to medical, behavioral health, and community appointments as needed, both during treatment and post-discharge
  • Provide transportation as needed
  • Facilitate life skills groups, supporting clients in developing practical skills related to daily living, self-advocacy, time management, community reintegration, and recovery stability
  • Communicate discharge and aftercare plans clearly with clients, families (as appropriate), and treatment team members
  • Collaborate with therapists, nursing, medical providers, admissions, and transportation partners to ensure timely and appropriate step-down placements and continuity of care
  • Maintain active communication with external providers, referral sources, transportation vendors, and community agencies
  • Ensure all discharge and case management documentation is completed accurately and within required timeframes
  • Track and follow up on outstanding referrals, appointments, transportation needs, and insurance-related requirements
  • Document all case management and coordination activities thoroughly in the electronic health record in compliance with regulatory and payer standards
  • Participate in multidisciplinary treatment team meetings and contribute to client-centered care planning

Minimum Qualifications
Education / Credentials
  • Associate's degree in Social Work, Human Services, Psychology, or a related field or CSAC-A (required)
  • Bachelor's degree in Social Work, Human Services, Psychology, or a related field (preferred)

Experience
  • Previous experience in case management, discharge planning, or care coordination
  • Experience working in behavioral health, substance use disorder treatment, or healthcare settings preferred

Knowledge / Skills
  • Strong understanding of discharge planning processes and continuity of care
  • Working knowledge of Medicaid enrollment processes and eligibility requirements
  • Excellent organizational, time management, and follow-up skills
  • Strong communication and interpersonal skills with clients, families, and interdisciplinary teams
  • Ability to work independently while managing multiple priorities
  • Proficiency with electronic health records and basic office software

Working Conditions
  • Direct care position with regular client interaction
  • Schedule aligned with program operations; flexibility may be required to meet discharge needs
  • 40 hours per week, Monday-Friday 8:00am-4:30pm Monday-Friday, with weekend rotation
  • Office-based with coordination across clinical units and external providers
