The Department of Economic Security, Division of Developmental Disabilities (DDD), is seeking an experienced and highly motivated individual to join our team as a Qualified Intellectual Disability Professional (QIDP). This position serves within a specialty unit of Support Coordinators who support individuals in State Operated Services ICF/IID (Intermediate Care Facilities for the Intellectually Disabled) settings. This position functions as a QIDP (Qualified Intellectual Disability Professional) for these settings as required by state and federal ICF standards.
DDD provides individuals with developmental disabilities, and their families, services and supports that are flexible, high quality, and member driven. These services provide individuals with opportunities to exercise their rights and responsibilities of independent decision-making and engagement in the community.
Applicants for this position cannot be licensed or certified by the DDD Office of Licensing Certification and Regulation, cannot have a principal interest in or have a spouse or close family with principal interest in a DDD Qualified Vendor Agreement, and cannot be employed or subcontracted by a DDD Qualified Vendor.
Job Duties:
Essential Duties and Responsibilities include but are not limited to:
• Coordinates and monitors long term care, physical health, and behavioral health services for members. Facilitates planning meetings, complete nursing assessments, active treatment observations, discharge planning, specialty care, arranging for medical equipment, promptly addressing changes in condition, coordinating physical health and behavioral health appointments and follow up.
• Provides oversight relating to services the member receives and reports quality of care concerns promptly per DDD's policies and procedures. Develop rapport and collaborate with residents, families and guardians, residential and day program staff, and medical professionals.
• Complete ongoing comprehensive progress notes that analyze the member's progress, regression, or maintenance of skills. Continuously evaluate the member's needs for services & supports, conduct referrals when a need is identified and ensure follow up is completed on recommendations for services, equipment, therapies, or prog. Monitor to ensure adequate svcs & supports are present to promote independence. Completes authorizations.
• Prepare a variety of reports and complete special projects. Attend staffing meetings, training and other meetings (including statewide travel). Confer with the supervisor.
• Ensure residential and day program staff are trained on the needs of the resident. Conduct incidental training as needed to ensure staff follow the Active Treatment program/ISP/therapeutic plans for each resident.
Knowledge, Skills & Abilities (KSAs):
Knowledge in:
• Methods, procedures, techniques, interdisciplinary team planning processes and person-centered planning strategies used in the development of Individual Support Plans
• Federal and State statutes and agency policies and procedures relating to the care and assistance for individuals with developmental disabilities and (ICF/IID) Intermediate Care Facilities for Individuals with Intellectual Disabilities
• Principles, practices and practices of case management & applied behavioral interventions.
• Community resources, medical and behavioral health issues and terminology
• Nursing theories and practices related to individuals with developmental disabilities and behavioral health conditions as well as managed care (Medicaid and Medicare) concepts
Skills In:
• Case Management (Coordinating and facilitating meetings Interviewing, Advocacy, Collaboration) in addition to identification of problems and their resolution
• Interacting in person, by phone and virtually with members, families, and representatives of other governmental and community organizations and the public
• Building consensus, continuous quality improvement and financial accountability
• Comprehensive needs assessments, nursing assessment, & needs analysis; in addition to excellent oral and written communication and ability to use computer software (e.g., Microsoft Word, Excel, databases, etc.)
• Initiating, developing, implementing, analyzing, monitoring and maintaining member treatment, active treatment and service plans
Ability to:
• Maintain confidentiality in addition to establishing and maintaining relationships
• Work independently and within diverse work teams including other physicians and health professional
• To think critically and problem solve in addition to being able to prioritize and multi-task to manage workload and meet timelines
• Observe and assess health status and needs of individuals with developmental disabilities and behavioral health conditions
• Interpret and apply DDD Policies and Procedures; and work within the legal system

Pre-Employment Requirements:
• If this position requires driving or the use of a vehicle as an essential function of the job to conduct State business, then the following requirements apply: Driver's License Requirements. Any employee operating a motor vehicle on State business must possess and retain a current, valid class-appropriate driver's license, complete all required training, and successfully pass all necessary driver's license record checks. The license must be current, unexpired, and neither revoked nor suspended. Employees who drive on state business are subject to Driver's license record checks, must maintain acceptable driving records, and must complete all required driver training. An employee who operated a personally owned vehicle on state business shall maintain the statutorily required liability insurance (see Arizona Administrative Code (A.A.C.) (R2-10-201.11).
• Employee shall have a valid Level One fingerprint clearance card issued pursuant to Arizona Revised Statute 41-1968 in order to work with children and vulnerable adults.
• Candidates for this position shall be subject to a search of both the Department of Child Safety Central Registry pursuant to A.R.S. 8-804 and the Adult Protective Services Registry pursuant to A.R.S. 46-459.
• Successfully pass background and reference check; employment is contingent upon completion of the above-mentioned process and the Agency's ability to reasonably accommodate any restrictions.

Retirement:
State employees are required to participate in the Arizona State Retirement System (ASRS), the State sponsored retirement contribution plan and the Long-Term Disability (LTD) program after a 27-week waiting period. The ASRS defined benefit plan provides for life-long income upon retirement.
