Job Description

We seek a highly motivated and organized Case Manager to join our team and help us achieve our mission. The Case Manager will assess, plan, coordinate, monitor, and evaluate the services required to meet an individual's health and human services needs. The Case Manager will work with clients, their families, and providers to develop and implement individualized service plans, ensuring the client receives the necessary support and resources.


Key Responsibilities:

- Conduct comprehensive assessments of clients' needs and develop individualized service plans.

- Coordinate and monitor the implementation of services and supports.

- Serve as a liaison between clients, families, service providers, and other stakeholders.

- Ensure compliance with DDD (Division of Developmental Disabilities) regulations and company policies.

- Maintain accurate and up-to-date case records and documentation.

- Conduct regular reviews and updates of service plans to reflect clients' changing needs.

- Advocate for clients' rights and needs within the community.

- Participate in team meetings and provide input on service delivery and program development.

- Assist with the annual criminal history check process and ensure all required documents are submitted and notarized.

Qualifications:

- Bachelor's degree in a related field or equivalent work experience.

- Proven experience in Case management or a related role.
