Position Summary

In person Office in Goodyear, Monday - Friday 8am - 5pm, training provided, company car provided for job required travel. 

Source of Hope LLC is seeking a compassionate, organized, and motivated Case Manager to join our growing team. The Case Manager plays a vital role in coordinating services, monitoring client progress, and ensuring individuals receive the supports they need to achieve their goals.

This position requires regular communication with clients, families, support coordinators, providers, and community partners. The Case Manager will conduct home visits, attend Person-Centered Service Plan (PCSP) meetings, monitor authorized service hours, and ensure services are delivered in accordance with Division of Developmental Disabilities (DDD) requirements and agency standards.

The ideal candidate is detail-oriented, proactive, and passionate about advocating for individuals with developmental disabilities.

Essential Duties and ResponsibilitiesCase Management & Client Support
  • Conduct comprehensive assessments of client needs and identify appropriate services and supports.
  • Develop, implement, and monitor individualized service plans.
  • Conduct regular home visits to assess client well-being, service effectiveness, and satisfaction.
  • Maintain ongoing communication with clients, families, guardians, and support networks.
  • Advocate for client needs and promote access to community resources and opportunities.
Service Coordination
  • Coordinate services among clients, families, providers, support coordinators, and community agencies.
  • Attend and actively participate in Person-Centered Service Plan (PCSP) meetings and other interdisciplinary team meetings.
  • Ensure services align with client goals, preferences, and DDD requirements.
  • Collaborate with providers to address service concerns and barriers to care.
Monitoring & Compliance
  • Monitor authorized service hours to ensure services are utilized appropriately and within approved allocations.
  • Track provider utilization and communicate with providers regarding available hours and service delivery expectations.
  • Review service documentation for accuracy, completeness, and compliance.
  • Ensure adherence to DDD regulations, HCBS standards, and company policies.
  • Conduct regular reviews and updates of client records and service plans.
Documentation & Administrative Responsibilities
  • Maintain accurate, timely, and confidential case records.
  • Prepare reports, assessments, meeting notes, and other required documentation.
  • Assist with annual criminal history checks and ensure all required employee and provider documents are completed, submitted, and notarized when necessary.
  • Support quality assurance and program improvement initiatives.
Team Collaboration
  • Participate in staff meetings, trainings, and professional development opportunities.
  • Provide input regarding service delivery improvements and client outcomes.
  • Work collaboratively with leadership and team members to support agency goals and mission.
QualificationsRequired Qualifications
  • Previous experience in case management, social services, healthcare, developmental disabilities, or a related field.
  • Knowledge of person-centered planning principles and community-based services.
  • Strong organizational, time management, and multitasking abilities.
  • Excellent verbal and written communication skills.
  • Ability to build positive relationships with clients, families, providers, and community partners.
  • Proficiency in Microsoft Office Suite, including Word, Excel, Outlook, and electronic record systems.
  • Strong problem-solving and critical-thinking skills.
  • Ability to work independently while managing multiple priorities.
  • Valid Arizona driver's license, reliable transportation, and proof of insurance.
  • Ability to travel locally for home visits, meetings, and client support activities.
  • Bilingual ( Spanish and English) 
Preferred Qualifications
  • Experience working with individuals with developmental disabilities.
  • Knowledge of Arizona DDD programs, HCBS requirements, and Medicaid-funded services.
  • Experience attending PCSP meetings and coordinating community-based supports.
