What does a day in this role look like?
We are looking for hardworking and compassionate caregivers wanting to enhance their skills, continue their education, and advance their career! You will be working one-on-one in-home with clients with disabilities to help them remain independent and comfortable in their own home. You will provide hands-on care, follow care plans, observe and document all client changes and cares. You will play an important role in ensuring that clients are provided with the best quality of care and remain a vital part of their community! Home Care Specialist Tasks (Including but not limited to):
  • Ensures continuity of care and meets clients' immediate needs by providing personal care and housekeeping tasks to clients as outlined on each client's care plan.
  • Respond to urgent service requests as directed to provide care to clients in their homes as outlined on each client's care plan.
  • Communicates regularly with the Care Solutions team on the status of client care and provides appropriate and timely documentation of daily tasks and client care needs.
  • Comply with CLA policies and procedures and various program and government regulations and rules by providing clear, accurate, and timely documentation and charting of all required information.
  • Acts as a resource for current caregivers by functioning as a peer mentor for new caregiver hires and providing training and job shadow opportunities.

Benefits:
  • Structured Pay Increases
    • Starting wage $18.00/ hr
    • Increase at 6 months of employment to $18.50/hr
    • Increase at 1 year to $19.00/hr
  • Wage Differential - Differentials can be combined (if applicable)
    • $0.50/hr between 6am-8am
    • $0.50/hr after 5pm
    • $1/ hr extra for hours worked on weekends

    • Certified Direct Care Professional (CDCP) (40 Hours of Mandatory Training)
      • Must be completed within the first 6 months of employment

Qualifications & Experience Requirements:
Required:
  • 18+ years of age
  • Authorized to work in the United States
  • Valid driver's license, reliable transportation, and current auto insurance to serve clients throughout Dane County
  • Capable of lifting/moving/pushing/pulling 35 pounds
  • Work around environmental hazards such as odors, second-hand smoke, animals, cleaning supplies, adverse living conditions, etc.
  • Work every-other-weekend and holidays
Preferred:
  • Own a Smart Phone for Electronic Visit Verification (EVV)
  • 1 Year of experience delivering personal cares in-home
