Position: Case Management Analyst

Location: Field Based - Arlington, Alexandria, Annandale, Springfield, Falls Church

Travel: Up to 75%

Position Summary
CaseManager Analystisresponsibleforfacetofaceandtelephonically assessing,planning,implementing,andcoordinatingallcase

management activitieswithmemberstoevaluatethemedicalneedsofthememberto facilitatethemember'soverallwellness.Developsaproactivecourseof actiontoaddressissuespresentedtoenhancetheshortandlong- term outcomesaswellasopportunitiestoenhanceamember'soverallwellness throughintegration.Servicesstrategiespoliciesandprogramsare comprisedofnetworkmanagementandclinicalcoveragepolicies.

Required Qualifications

  • Must reside in Annandale, Falls Church, Springfield, Arlington, Alexandria VA area
  • Must possess reliable transportation and be willing and able to travel up to 75% of the time. Mileage is reimbursed per our company expense reimbursement policy
  • 2 or more years' experience in behavioral health, social services or appropriate related field


Preferred Qualifications

  • Case Management, Discharge Planning and/or Managed Care Experience


Education

  • Bachelor's Degree or non-licensed master level clinician required, with either degree being in behavioral health or human services preferred (psychology, social work, marriage and family therapy, counseling), LMHP, RN/LPN, QMHP, LMSW, LBSW, MSW or BSW.

The typical pay range for this role is:

$21.10 - $40.90

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
