LSW and or CSW Social Worker

About The Role:

As an LSW and/or CSW , you will be responsible for providing social work services to our residents, including assessment, counseling, and case management. You will work closely with our healthcare team to develop and implement individualized plans to meet the needs of our residents. Your strong communication and interpersonal skills will be essential in building relationships with residents, families, and staff.

Key Responsibilities:

โ€ข Conduct initial and ongoing assessments of residents' social, emotional, and psychological needs
โ€ข Develop and implement individualized care plans to address residents' needs and goals
โ€ข Provide counseling and therapy services to residents, including group therapy and support groups
โ€ข Collaborate with interdisciplinary teams to address residents' physical, emotional, and social needs
โ€ข Coordinate with community resources and agencies to provide additional support services to residents
โ€ข Maintain accurate records and documentation of resident services and progress
โ€ข Participate in quality improvement initiatives to improve resident outcomes and services

Requirements:

โ€ข LSW and/or CSW licensure in the state of NJ
โ€ข Bachelor's degree in Social Work or related field
โ€ข 3+ years of experience in social work, gerontology, or a related field
โ€ข Strong communication and interpersonal skills
โ€ข Ability to work independently and as part of a team
โ€ข Knowledge of aging issues, long-term care, and social work principles and practices
