We are seeking a dedicated Social Worker (LSW/CSW) to join our team in a temporary full-time capacity. As a Social Worker (LSW/CSW), you will play a vital role in supporting residents and their families while working alongside an interdisciplinary team committed to delivering high-quality care.

Social Worker (LSW/CSW) Qualifications & Responsibilities:
  • Must possess a current New Jersey LSW or CSW license.

  • Previous subacute and/or long-term care experience preferred.

  • Complete resident Social Histories and Psychosocial Assessments.

  • Help identify residents' psychosocial, emotional, and mental health needs.

  • Collaborate with the interdisciplinary team to promote and protect resident rights.

  • Prevent, recognize, and address resident abuse in accordance with applicable laws and professional standards.

  • Maintain accurate documentation and resident records.

  • Experience with PointClickCare electronic medical records preferred.

  • Ability to build and maintain positive relationships with residents, families, and team members.

  • Ability to work independently and as part of a team.

  • Perform other duties as assigned by the Director of Social Services and Administrator.
