Position Title: Patient Logistics RN or EMT or Paramedic

Schedule:

  • 36 hours per week, night shift

  • weekend and holiday rotation

POSITION PURPOSE

The Patient Logistics RN serves as the primary clinical coordinator for complex patient transitions across the healthcare continuum. This role managesinterfacilitytransfers by integrating advanced clinical assessment, situational awareness, andrealtimedecision-making to ensure patients are placed at theappropriate levelof care ina timelyand safe manner. Acting as the first clinical point of contact forreferringhospitals, providers, and healthcare facilities, the RN evaluates patient acuity, determinesappropriate clinicalpathways, andanticipatespotential risks during transfer or transport.ThePatient LogisticsRN functions as a critical communication hub-linking referring providers, accepting clinicians, receiving units, transport teams, and family members-to ensureaccurate, comprehensive, and clinically relevant information flows between all parties. This role requires strong clinical judgment, rapid prioritization skills, and the ability to navigate bed capacity constraints, operational limitations, and emergent care needs.Through diligent attention to regulatory requirements, specifically EMTALA and other transfer-related standards, thePatient LogisticsRN upholds patient safety, compliance, and continuity of care. Byoptimizingresources,advocating forpatients, andfacilitatingefficient transitions, this role significantly influences patient outcomes, throughput performance, and overall healthcare system efficiency.

ESSENTIAL FUNCTIONS

1. Facilitate and coordinate patient transfers between healthcare facilities, departments, or units2. Work in a fast-paced, ever changing working environment.3. Conduct clinical assessments of transfer requests to determine appropriate level of care needed4. Communicate effectively with physicians, nurses, and staff at both sending and receiving facilities5. Manages multi-phone call activity to minimize any delays experienced by customers6. Document all transfer information accurately in electronic health records and transfer systems7. Coordinate transportation arrangements including ground and air ambulances when necessary8. Monitor bed availability and collaborate with bed management teams9. Serve as a clinical resource for transfer-related questions and concerns10. Participate in quality improvement initiatives related to transfer processes 11. Demonstrates knowledge of the use of emergency notification system (PULSARA) and takes initiative to activate system for emergency transfer of patients with STEMI, stroke, trauma, ECMO.12. Takes the initiative to anticipate problems, uses creative thinking and generates reports as requested.

MINIMUM QUALIFICATIONS

1. Must possess a comprehensive understanding of the Paramedic scope of practice, including all clinical procedures, patient care standards, and legal/regulatory requirements governing emergency medical services. This knowledge should encompass state and local protocols, documentation standards, infection control practices, and the ability to apply these principles effectively in both pre-hospital and hospital settings. Additionally, the Paramedic should demonstrate proficiency in recognizing limitations of the role and collaborating with other healthcare professionals to ensure safe, high-quality patient care.2. High school diploma or equivalent (GED).3. Graduate of an approved Paramedic course with a current Iowa Paramedic (PM) certification, or an Iowa RN Exemption or Illinois PHRN certification, based on role. (MercyOneGenesis - current Paramedic (PM) license in either Iowa or Illinois isrequiredat the time of hire. A secondary PM license for the other state must be obtained within 120 days of the hire or transfer date.)4. Minimum of 2-3 years of clinical experience.5. Must be comfortable operating in a collaborative, shared leadership environment.6. Mustpossessa personal presence that is characterized by a sense of honesty, integrity, and caring with the ability to inspire and motivate others to promote the philosophy, mission, vision, goals, and values of Trinity Healthand MercyOne.7. Strong verbal, written, and interpersonal communication skills.8. Excellent organizational, prioritization, and multitasking abilities in a fastpaced environment.9. Proficiency with electronic health records and transfer management systems.10. Working knowledge of Microsoft Word, Excel, and Google-based applications.
