Constructive Partnerships Unlimited (CP Unlimited) is a comprehensive multi-service and multi-site organization providing services and programs to people with intellectual and developmental disabilities throughout the NYC metropolitan areas. Â
CP Unlimited operates an extensive community based living program, day habilitation programs, community habilitation services and provides rehabilitative services in a caring and comfortable atmosphere. CP Unlimited also provides nursing, behavioral, dietary and Article 16 services including counseling, physical therapy, occupational therapy, and speech therapy. Well- coordinated care, overall wellness, and building personalized relationships with the people that reside and participate in our programs are our high priorities.
Currently CP Unlimited is seeking a full time Physical Therapist in our Bronx Clinic and Residential Community Program to plan and implement physical therapy treatment programs to maintain and/or restore function secondary to acute or chronic disability.
Specific Position Duties Include:
- Reviews doctor referrals and medical records to determine PT treatment Refers to specialty clinics as needed (adaptive equipment, shoe and splint). Â Maintain active/up to date MD prescriptions as outlined by NYS Department of Health (DOH) and Office for People with Developmental Disabilities (OPWDD) depending upon work assignment.
- Evaluates and assesses the needs and level of functioning in the areas of: sensorimotor, gross & fine motor skills, musculoskeletal status, CNS status, ambulation and adaptive equipment needs (e.g. walker, canes, AFO's).Â
- Devises treatment plans based on the assessment; administers treatment involving the application of modalities: hydrotherapy, hot packs, cold packs, ultrasound, and electrical stimulation; instructs, motivates, and assists persons receiving services in various exercise programs utilizing weights, exercise machines, mat activities, assistive devices, orthoses, and prostheses.
- Provides training as needed.
- Participates and attends care plan/team meetings as needed for persons receiving PT services.
- Supervises PTAs & therapists on limited permits, and performs oversight in the Healthcare Center, day programs and residential settings.Â
To qualify for the Physical Therapist Position you must possess the following:
- Graduate of an accredited physical therapy school with a Bachelor's Degree or higher.Â
- NYS Physical Therapy License. Â
- Knowledge in PT evaluation and treatment; ability to supervise direct assistants and aides. Understand the principles of physical agent modalities employed in PT.Â
- Knowledge in the use of mobility devices and therapeutic exercises.Â
- Must have good written-verbal communication skills.Â
- Proficient with Microsoft Office.
- Able to travel as assigned to oversee PTAs in the Healthcare Centers, day programs or residential sites.
