Cross Country Physical Therapy is seeking a dedicated and skilled Full-Time Home Care Physical Therapist. In this role, you will evaluate and provide treatment for patients recovering from injuries, surgeries, or chronic conditions in the comfort of their own homes. This position is primarily onsite, where you will provide home care services tailored to the individual needs of our patients. Key Responsibilities
  • Conduct thorough evaluations of patients in a home care setting to assess their physical capabilities and limitations.
  • Develop, implement, and adjust individualized treatment plans that utilize various methodologies, including exercise, manual therapy, and modalities to reduce pain and enhance mobility.
  • Document all treatment procedures, patient responses, and progress notes meticulously to track outcomes and adjust plans as needed.
  • Regularly reevaluate therapeutic goals, ensuring treatment plans evolve based on patient needs and progress.
  • Educate patients, caregivers, and families on effective physical therapy practices and the use of assistive devices to promote independence.
  • Engage in continuous professional development by participating in workshops, reading relevant literature, and networking with other experts in the field.
Patient Population / Client Focus

As a Home Care Physical Therapist, you will work with a diverse patient population, including children, adults, and seniors with various diagnoses, such as post-surgical rehabilitation, chronic pain, and mobility challenges. Your approach will be tailored to the unique circumstances and preferences of each patient, allowing you to provide holistic and integrated care that addresses both physical and emotional healing.

Qualifications
  • A Doctorate degree in Physical Therapy (DPT) is required.
  • Current and valid State License in Physical Therapy.
  • A minimum of two years of experience in a clinical setting, particularly in home health or outpatient physical therapy.
  • Strong verbal and written communication skills to effectively interact with patients, families, and the healthcare team.
  • Proficiency in basic computer applications and electronic documentation systems.

Preferred Experience

Compensation and Benefits

The salary for this position ranges from USD 90,000 to USD 110,000 annually, commensurate with experience and qualifications.
