What Does a Correctional Case Manager Do?
A correctional case manager works with convicted criminals to help them re-enter society as productive citizens. As a correctional case manager, you act as a liaison between social services and the correctional system. Part of your job duties include working with inmates while they are finishing their time in prison to help identify skills they can use to find gainful employment upon release. You also work with social services to ensure that the inmates will be getting the appropriate treatment once they are outside of prison. Experience in social sciences and a knowledge of the prison system are important.

Summary Statement
The Fair Hearing Team is seeking a Health/Human Services Case Manager II (H/HS CM II) for the Blue Hen Office. You will be responsible for ensuring actions proposed by DSS and DMMA are correct and accord with applicable regulations. Conduct research, coordinate corrections, communicate information to internal/external stakeholders, write summaries, and represent the agency in administrative hearings. Timeliness and accuracy requirements are controlled by state and federal regulations.\r\nYou will support the administrative hearing process across all programs administered by the Agency. Statewide travel will be required as training for this position and any in-person meetings will be conducted in New Castle County. Your presence for in-person hearings in Sussex County may be required.\r\n\r\n
Essential Functions

An employee in this class may participate as a member of a multi-disciplinary team to implement behavioral health services or treatment plans in accordance with Delaware Code, Title 24.\r\n\r\n\r\n\r\n\r\n Interviews client, family, or professional sources to determine the client's needs for a variety of health and human services which may include employment, job search, and basic skills remediation.\r\n Develops a comprehensive cost-effective service plan utilizing in-house sources, referrals, contracted providers, or institutions (Service plans may include job training and employability, housing, general public assistance, in-home services and supports or home-delivered meals, de-escalation techniques, peer services, or skill development).\r\n Conducts ongoing monitoring and assessment of service delivery for optimum quality and efficiency; authorizes and recommends necessary changes and adjustments.\r\n Maintains comprehensive case records, summaries, statistical and narrative reports, and completes required forms.\r\n Establishes effective working relationships with clients, family, community, and professional sources.\r\n Provides guidance and relevant information to connect clients with needed services which may include protective, adoption, and related services.\r\n Provides information and case coordination on-site, via email or telephone to individuals or groups.\r\n Supplies information and responses to inquiries regarding services for clients, family, community, and government agencies.\r\n May provide supportive casework to clients prior to, during, or after hospitalization based on physician diagnosis and recommended treatment.\r\n Uses automated information systems and computer software to research, review, evaluate, and report on health/human service data.\r\n Communicates and interacts effectively with people across cultures, ranges of ability, genders, ethnicities, ages, and races.\r\n Performs other related duties as required.\r\n\r\n\r\n

JOB REQUIREMENTS forHealth/Human Service Case Manager II\r\nApplicants must have education, training and/or experience demonstrating competence in each of the following areas: \r\n\r\n Six months' experience in case management which includes assessing, planning, developing, implementing, monitoring, and evaluating options and services to meet an individual's human service needs. \r\n Six months' experience in making recommendations as part of a client's service plan such as clinical treatment, counseling, or determining eligibility for health or human services/benefits. \r\n Six months' experience in interpreting laws, rules, regulations, standards, policies, and procedures.\r\n Six months' experience in using an automated information system to enter, update, modify, delete, retrieve/inquire and report on data. \r\n Six months' experience in narrative report writing. \r\n\r\n
