Part-Time I First Shift
Hospital Pharmacy I Corbin, KY
The pharmacy department works to achieve optimal medication use and patient outcomes in a collaborative, patient-focused environment. Pharmacy is responsible for the storage, preparation (including sterile compounding), distribution, and dispensing of medications 24 hours a day, seven days a week. Pharmacy utilizes automated dispensing cabinets and barcoding to enhance patient safety and operational efficiency. Inpatient pharmacy clinical services involve pharmacists working directly with healthcare teams to optimize patient drug therapy by providing drug information and therapeutic monitoring.
Description:
  • Serves as a primary point of contact with patients related to specialty medication needs and demonstrates empathy and compassion to patients and caregivers.
  • Enrolls specialty pharmacy patients into medication management programs by assisting with recruitment of patients in clinic.
  • Provides initial clinical medication counseling and regular patient reassessments via in person, telephonic, or video encounters to ensure appropriateness of specialty therapy and ongoing therapy management.
  • Performs medication therapy management, which may include coordinating medication adjustments to optimize patient medication regimens and improve clinical outcomes.
  • Refers appropriate patients to financial counseling for copay assistance through manufacturers or charity programs.
  • Communicates with patients and families about the benefits of filling their prescriptions with Baptist Health.
  • Coordinates drug distribution in accordance with scope of practice, policies and procedures, gender and age of patient served.
  • Collaborate with providers and clinical staff to integrate pharmacy services and coordinate care with eligible patients.
  • Communicates with physicians and other clinicians as necessary to facilitate exemplary service for new and existing Baptist Health specialty pharmacy patients.
  • Provides limited/indirect supervision for Specialty Pharmacy Care Coordinators and timely response to clinical escalations.
  • Maintain pharmacy onsite clinic services, operational policies and procedures, clinical pharmacy workflows and practice expectations.

Requirements:
  • Bachelor's Degree Pharmacy.
  • Kentucky Board of Pharmacy license in good standing as applicable to practice site.
  • 1-3 years related work experience or completion of ASHP accredited pharmacy residency program preferred.
