Care Coordinator Job Description

REPORTS TO: Care Coordination Manager

POSITION SUMMARY:

Responsible for essential administrative tasks in relation to Care Coordination.

QUALIFICATIONS:

• High School diploma or GED, Associates degree preferred.

• Must demonstrate excellent interpersonal skills; both written and verbal, time management, and

Microsoft Office skills.

• Be self motivated and should need minimal supervision.

• Be detail oriented and organized.

• Ability to multi-task competing priorities.

• Minimum of one (1) year experience in a medical office environment.

ESSENTIAL FUNCTIONS/AREAS OF ACCOUNTABILITY

1. Effectively manage and organize the Care Coordination spreadsheet.

2. Gather, collect, and document coordination of multi-disciplinary services.

3. Accurately complete and upload care coordination forms into the patient chart in the EMR.

4. Manage incoming and outgoing faxes and emails in relation to care coordination.

5. Effectively prioritizes required tasks with clinical staff.

6. Communicate effectively with all members of the interdisciplinary team through verbal reports,

participation in staff meetings, and team conferences, as requested.

7. Understands regulations and requirements for Medicaid Home Health services.

8. Performs other related duties and responsibilities as deemed necessary
