Job description

Arista Healthcare is hiring a compassionate Registered Nurse (RN) Case Manager to provide Hospice nursing care in the Tucson area. This role is ideal for an RN seeking flexible scheduling and meaningful one-on-one patient care. Hospice RN experience preferred, but not required — we will train.

RN Case Manager – Job Summary

The RN Case Manager provides skilled nursing services to patients receiving Hospice care in their homes. The RN Case Manager coordinates care, completes patient assessments, develops plans of care, and collaborates with physicians and the interdisciplinary team to ensure high-quality patient outcomes.

Registered Nurse (RN) Responsibilities

  • Perform comprehensive hospice RN assessments
  • Provide skilled nursing care including medication management, wound care, symptom management, and patient education
  • Develop, implement, and update plans of care as an RN Case Manager
  • Coordinate care with physicians, social workers, aides, and therapists
  • Educate patients and families on disease process, treatment plans, and end-of-life care when applicable
  • Complete accurate and timely documentation in the electronic medical record (EMR)
  • Participate in on-call rotation as assigned (hospice roles)
  • Ensure compliance with state, federal, and CMS regulations for home health and hospice

RN Qualifications

  • Current Arizona Registered Nurse (RN) license
  • Valid driver’s license and reliable transportation
  • Ability to work independently in the home setting
  • Strong communication, organization, and time-management skills
  • Compassionate, patient-centered approach to nursing care
