Our Outreach & Enrollment team serves as a trusted guide, helping community members understand their options, secure healthcare coverage, and connect with essential resources that support healthier lives and brighter futures.

What Success Looks Like

This is a community-facing role that combines outreach, education, enrollment assistance, and benefits navigation. Coordinators must be approachable, organized, and committed to helping individuals overcome barriers to healthcare access. Success in this role means building trusted relationships in the community, accurately guiding clients through complex enrollment processes, resolving coverage barriers, and ensuring individuals and families remain connected to the healthcare services they need. You'll help expand access to care, improve continuity of coverage, and support positive health outcomes for vulnerable populations.


The Role

  • Conduct outreach to individuals, families, and community partners to increase awareness of available health coverage programs and services
  • Provide educationregardinghealth insurance options, eligibility requirements, enrollment processes, and available community resources
  • Participate in outreach events, health fairs, enrollment activities, and community engagement efforts
  • Conduct eligibility screenings andassistclients with enrollment in Medi-Cal, Covered California, managed care plans, and other available health coverage programs
  • Complete applications, gather required documentation, and help resolve barriers that may delay or prevent enrollment
  • Monitor application status, renewals, redeterminations, and follow-up activities to support continuity of coverage
  • Coordinate with clients, county eligibility workers, managed care organizations, healthcare providers, and community partners to address enrollment issues
  • Connect clients to supportive services, including transportation, nutritionassistance, housing resources, and other community-based programs, as appropriate
  • Maintaintimely,accuratedocumentation of outreach, enrollment, and client engagement activities
  • Support care coordination efforts and collaborate with interdisciplinary teams to promote access to healthcare services

Desirable Candidate Profile

  • High School Diploma or GEDrequired
  • Minimum two (2) years of experience in health insurance enrollment, eligibility determination, patient financial services, benefits navigation, care coordination, customer service, or a related healthcare or human services role; or an equivalent combination of education and experience
  • Ability to obtain andmaintainCertified Enrollment Counselor (CEC) certification within90 daysof hire
  • Experience working with Medi-Cal, Medicare, Covered California, managed care organizations, or public benefit programs strongly preferred
  • Experience working with individuals experiencing financial hardship, housing instability, or other barriers to healthcare access preferred
  • Bilingual English/Spanish required

Work Schedule

This is a full-time position that combines office-based work, community outreach, enrollment support, and participation in outreach events throughout the community. While the typical schedule is Monday through Friday, occasional evenings or weekends may be required to support outreach activities, enrollment events, community partnerships, and operational needs.


Compensation & Benefits

The anticipated starting pay range for this role is $22.00 to $25.30 per hour, paid out bi-weekly. Final placement and starting pay rate will be determined based on the candidate's experience, qualifications, certification status, and internal equity considerations.
