Job description

The Primary Care Physician provides comprehensive, patient-centered medical care for individuals of all ages in a community health setting. Responsibilities include preventive care, diagnosis and management of acute and chronic conditions, and coordination with other healthcare professionals to ensure integrated, high-quality services. The physician collaborates with a multidisciplinary team and supports the FQHC's mission to deliver accessible, person-focused care to the community.
Duties/Responsibilities:
Below are the primary duties and essential functions of an employee in this position. While not all tasks may be performed by every employee in this role, all employees are expected to perform related tasks as needed. These essential functions must be performed with or without reasonable accommodation.
Clinical:

  • Collect, record, and maintain patient information.
  • Prescribe or administer treatment, medical therapy, medication, vaccination and other specialized psychiatric medical care to treat or prevent illness, disease or injury.
  • Order, perform and interpret tests and analyze records, reports, and examination information to diagnose a patient's condition.
  • Monitor patient's conditions and progress and reevaluate treatments as necessary.
  • Explain procedures and discuss test results or prescribed treatments with patients.
  • Refer patients to medical specialists or other practitioners as necessary.
  • Other duties as assigned.
Administrative:
  • Attend and contribute to monthly medical staff meetings.
  • Complete Nurse Practitioner and Physician Assistant peer reviews.
  • Complete documents requested by third-party pay sources.
  • Complete documentation timely.
  • Supervise NP's, PA's and Advanced Practice Nurses as assigned.
  • Maintain CEUs required for continued medical license in Indiana.
  • Maintain Drug Enforcement Administration (DEA)
  • Maintain credentials with all applicable managed care contracts by completing the
  • necessary paperwork and returning it on a timely
  • Other duties as assigned.
Physical Requirements:
  • Occupational Exposure Risk Classification: The normal work routine involves moderate exposure to blood, body fluids, or tissues.
  • The normal work routine has a minimal ergonomic risk for repetitive motion impairment.
  • The position may require a minimal amount of physical exertion (bending, lifting, twisting) in the work area.
Required Qualifications:
  • Education: D., D.O. required.
  • Certificate or Professional License: Current Board Certification in Internal Medicine, Family Practice, or Pediatrics, or eligible to be Board certified.
  • Prescriptive Authority in Indiana (DEA License).
  • CPI (Crisis Prevention Intervention) - certification must be obtained after hire and prior to providing direct patient care. This certification must be maintained and kept current through employment.
  • BLS (Basic Life Support) and First Aid - certification must be obtained after hire and prior to providing direct patient care. This certification must be maintained and kept current through employment.
  • Valid driver's license and active auto insurance coverage.
  • Satisfactory results from criminal and driving record background checks.
