Job Title: Public Health Nurse (PHN)

Place of Work: Toppenish, WA
Site: Yakama Indian Health Center (YIHC), Portland Area IHS – Yakama Service Unit
Location: 401 Buster Road, Toppenish, WA 98948, plus community outreach events within the Yakama Nation Reservation.
Duration: 1 year contract with the possibility of 4 optional years
 
Shift Schedule: Day shift only, Monday through Friday, 8:00 a.m. to 5:00 p.m. No nights, weekends, or federal holidays.
Credentialing / Onboarding Time: 6 to 8 weeks
License: Current, unrestricted RN license - any U.S. state accepted
Certifications: Current BLS
Education: BSN required - bachelor's degree in Nursing from a State-approved professional nursing program
Experience Needed: Minimum 1 year of public health nursing or community outreach experience
Special Skills Needed: Must demonstrate knowledge of communicable disease control and contact tracing protocols; ability to coordinate with public health entities and interdisciplinary teams; valid driver's license and vehicle for field/outreach visits
Responsibilities:
  • Conduct case management, outreach, and contact tracing for communicable diseases (e.g., tuberculosis, STIs, measles).
  • Perform directly observed therapy (DOT), documenting completion daily on clinic days.
  • Coordinate with local, state, and federal public health departments.
  • Review and act on lab results, and provide patient counseling and education.
  • Administer medications and immunizations, and conduct field visits for community follow-up.
  • Develop and deliver public health education sessions.
  • Document all activities in the EHR or approved public health systems.
  • Drive to outreach and community activities as needed.
