Join Our Team as a Human Services Case Manager!

We are seeking a caring and dynamic Human Services Case Manager to join our team for an immediate opening. If you are passionate about advocating for others and helping individuals navigate their care, we encourage you to apply.

What You'll Do:

  • Act as a central point of contact and support for Participants in Kentucky Waiver Programs, ensuring they have access to the resources and services they need.
  • Create and manage personalized care plans that are respectful of each participant's goals and preferences.
  • Build strong, positive relationships with participants, their families, and a network of service providers.
  • Complete required training to maintain a high level of competency and compliance, including topics like trauma-informed care, abuse and neglect reporting, professional boundaries, and person-centered thinking, and incident reporting.
  • Engage in local travel (approximately 50%) to conduct in-person visits and attend necessary appointments.

Preferred Qualifications:

  • 4+ years of professional experience in KY Waiver Programs as a Case Manager, or in a human services, healthcare, or related field is highly preferred.

Minimum Qualifications: You must possess at least one of the following qualifications:

  • A Bachelor's degree in Social Work, Human Services, or a related field.
  • A Bachelor's degree in any field, complemented by at least one year of Human Services experience.
  • An Associate degree in a Behavioral or Social Science field, with two years of Human Services experience.
  • A Kentucky-licensed RN with two or more years of experience in the field of aging or disabilities.
  • A Master’s degree in a Health or Human Services field from an accredited college or university.
