We are seeking an additional Full-Time Adult Case Manager for our Ellsworth Outpatient Office.


Position Summary

The Adult Behavioral Health Home Coordinator / Community Integration Specialist supports adults in building the skills and accessing the resources needed for successful, independent living. This role emphasizes holistic, person-centered care guided by SAMHSA's 8 Dimensions of Wellness-emotional, social, physical, financial, and more.


You'll work as part of a multi-disciplinary team, using psychiatric rehabilitation and integrated care models to support both mental and physical health outcomes.


What You'll Do
  • Build strong, therapeutic relationships with clients-including those who may be harder to engage
  • Help clients set and achieve personalized wellness goals
  • Provide skill-building and psychoeducational support (individual & group settings)
  • Coordinate care plans and connect clients to essential community resources
  • Support clients and families in making informed decisions
  • Assist with access to medical, behavioral health, and social services
  • Provide crisis intervention when needed
  • Maintain accurate documentation and meet service delivery standards

Work Environment

  • Occasional evenings or extended hours as needed
Physical Requirements
  • Ability to sit, stand, walk, and use standard office equipment
  • Occasionally lift up to 25 lbs
  • Travel within the service area required
