Community Liaison

Shift: Mon to Fri, 8 am to 5 pm onsite, with occasional evening or weekend hours required.

Status: Full time

Position Purpose:

Are you passionate about helping seniors live safely and independently while building strong community relationships?

We are seeking a motivated, relationship-driven Community Liaison to grow awareness and referrals for our PACE program. This role is ideal for someone with healthcare sales, senior services, outreach, and provider relations experience.

In this position, you'll build referral partnerships, educate the community, and connect eligible seniors with life-changing care and support services.

What you will do:

  • Initiate, build relationships with and maintain contact with community referral sources to promote the PACE program.

  • Develop and maintain referral relationships with hospitals, physicians, rehab centers, senior centers, and community organizations

  • Educate referral sources, families, and community partners on program benefits

  • Generate qualified referrals and support enrollment growth

  • Conduct presentations, community events, and outreach activities

  • Maintain territory plans and referral pipelines

  • Collaborate with enrollment, clinical, and operations teams to ensure smooth participant transitions

Minimum Qualifications:

  • Bachelor's degree with 2+ years of experience in healthcare sales, outreach, business development, or community engagement. A combination of education and experience may be acceptable.

  • Experience in senior care, home care, hospice, PACE

  • Strong networking and relationship-building skills

  • Excellent communication and presentation skills

  • Self-motivated with ability to manage a territory independently

  • Comfortable in assisting seniors with the process in their homes

  • Valid driver's license and reliable transportation

Preferred Backgrounds:
