Job description

The Coordinated Entry System Coordinator facilitates Champaign County’s Coordinated Entry System (CES). The Coordinator ensures efficient and effective responses and services to individuals and families experiencing homelessness in alignment with HUD requirements and Champaign County Continuum of Service Providers to the Homeless (CSPH) standards. Core responsibilities include managing the Coordinated Entry System in partnership with CSPH and RPC leadership, as well as providing direct Coordinated Entry services to individuals and families. The Coordinator enters, maintains, and regularly reports on Coordinated Entry System data; recruits, trains, and supports CES partner organizations; facilitates regular CES committee meetings; supports grant applications and reports; develops expertise in CES; and increases awareness and understanding of CES within the community through outreach.

Additional duties include conducting timely assessments, prioritizing client needs, connecting individuals and families to appropriate housing and supportive services, coordinating with service providers to maintain the by-name priority list, recertifying homelessness status, facilitating housing and homeless services referrals, and reducing barriers to housing.

 

  • Bachelor’s degree in human services or related field and a minimum of three years of direct client service experience; combination of education, training, and experience in the field may be considered.
  • Ability to pass criminal background check.
  • Strong background in customer service and human services with program’s target population. Preference given to those who have experience with homelessness/crisis intervention work.
  • Ability to work outside of traditional business hours to accommodate program needs, including periodic early mornings, evenings, nights and weekends.
  • Valid driver’s license and safe and reliable vehicle. Ability to transport clients using agency vehicles on occasion.
  • Ability to visit clients in community-based sites in Champaign County (e.g., libraries, shelters, public building, etc.).
  • Ability to follow program procedures while adjusting work style to meet the needs of the client.
  • Demonstrate excellent organizational skills with experience accurately documenting services through case notes, maintaining spreadsheets, and managing competing priorities highly desired.
  • Knowledge of housing continuums from shelter to home ownership in service area.
  • Proficient basic to advanced math skills to complete financial forms, develop reports and understand statistical data. Must use math in daily work tasks, such as calculating financial eligibility for services, determining level of subsidy assistance, and monitoring expenditures.
  • Knowledge of public assistance programs, including eligibility and application process.
  • Ability to attend community meetings, provide program information at community meetings, and collect and share relevant meeting information with team.
  • Demonstrate excellent listening skills, presentation and public speaking skills, and the ability to share information with diverse audiences.
  • Strong interpersonal, written, and verbal communication skills and the ability to forge positive relationships with clients, co-workers, and colleagues.
  • Ability to use organization and records management skills to manage all aspects of a client’s case, including tracking activities, updating records, monitoring outcomes, managing files and documents, and following all recordkeeping requirements.
  • Proficient in English (spelling, punctuation, grammar).
  • Ability to attend worksite regularly to perform job duties and responsibilities.
  • Aptitude to become proficient with proprietary database operations and demonstrate functional proficiency in file management and use of Microsoft Office Suite (Word, Excel, Outlook, PowerPoint) to manage computer files, use databases, enter data, create spreadsheets, and prepare presentations.
  • Ability to obtain program specific certification(s) within twelve months. This may include 40-hour domestic violence professional training.
  • Ability to attend program meetings and/or training as required.
  • Preferred: Knowledge of Coordinated Entry Systems and Continuums of Care.



Schedule:   8:00 a.m. – 4:30 p.m., M-F (Flexibility will be necessary to support the goals of the program.)
