Job description

We are seeking a detail-oriented and compassionate Admissions Coordinator to join our team at a skilled nursing facility in Cobble Hill, Brooklyn. This role is essential in ensuring a smooth and welcoming transition for new residents and their families while supporting census growth and maintaining strong referral relationships.

Key Responsibilities

  • Manage the full admissions process from initial inquiry through admission
  • Serve as the primary point of contact for hospitals, families, and referral sources
  • Coordinate and conduct facility tours for prospective residents and families
  • Review referral documentation to determine eligibility and appropriateness for admission
  • Collaborate with interdisciplinary team members to ensure seamless admissions
  • Maintain accurate records in EMR systems and track admissions data
  • Verify insurance coverage and coordinate with billing as needed
  • Follow up on pending referrals and maintain strong communication with referral partners
  • Participate in marketing and outreach efforts to support census development

Qualifications

  • Previous experience in admissions, healthcare marketing, or case management preferred
  • Experience in a nursing home or healthcare setting strongly preferred
  • Knowledge of Medicare, Medicaid, and managed care processes
  • Strong interpersonal and communication skills
  • Excellent organizational skills and attention to detail
