Our interdisciplinary team works collaboratively to ensure the highest quality outcomes for those we serve.

Position Summary

We are seeking a compassionate and detail-oriented Registered Dietitian (RD) to join our team. The ideal candidate will be responsible for assessing residents' nutritional needs, developing individualized care plans, and ensuring compliance with all regulatory standards.

Key Responsibilities

  • Conduct comprehensive nutritional assessments for residents upon admission and at regular intervals
  • Develop, implement, and monitor individualized nutrition care plans
  • Collaborate with physicians, nursing staff, and interdisciplinary team members
  • Monitor residents' weight, lab values, and dietary intake
  • Provide nutrition education and counseling to residents and families
  • Ensure compliance with state, federal, and facility regulations
  • Participate in care plan meetings and quality improvement initiatives
  • Oversee food service operations in collaboration with dietary staff as needed

Qualifications

  • Registered Dietitian (RD) credential required
  • 3-4 years of Experience as a RD in a SNF required.
  • Bachelor's degree in Dietetics, Nutrition, or related field
  • Current state licensure (or eligibility) in New York
  • Previous experience in long-term care or clinical setting preferred
  • Strong communication and organizational skills
  • Knowledge of regulatory standards (DOH, CMS)
