The Program Coordinator of Intensive Community Services supervises the Alternative Care/ Nursing Home team, which is comprised of a team of mixed licensed clinicians (BA, LCSW, LPC, MSW) who serve the SPMI (severe and persistent mental illness) population in the community, along with Hospital Liaison Team, who ensure care coordination during inpatient psych hospitalizations.ย  Beyond providing clinical supervision, the coordinator also maintains relationships with the 30+ community ACFs and nursing homes that JCMH partners with. ย This position is great for anyone who has clinical experience with the SPMI population and is looking to take on a leadership/supervisory role with a well-established and seasoned team. ย 

Education, Knowledge, Skills & Experience Required:
  • Masters Degree in related field (social work, counseling, etc.) required
  • Licensure (LPC, LCSW, LMFT, etc) required
  • CAC II or III preferred, but not required
  • Bilingual (English/Spanish) preferred
  • Minimum of two years experience in program management or in direct supervision of staff is necessary for this position.
  • Knowledge of 27-65 (mental health certification) process
Essential Duties:
  • Supervision of ACFNH Case Managers
  • Supervision of Hospital Liaisons
  • Coordinate new referrals into ACFs and placements with internal/external staff including community agencies, and Jefferson Center staff
  • Liaison for Jefferson Center with Nursing Home staff, ACF providers, including quarterly psycho-education, training, and coordination meetings
  • Advocating for the needs of the team and the SPMI population
  • Documentation in timely manner within the standards of Jefferson Center
  • Coordination of care with consumers and facility
  • Audit team charts for compliance of 27-65 paperwork, documentations and other BHA and Jefferson Center requirements
  • Hiring, Training, Performance Evaluations of supervisees
  • Training/education for staff (internal/external) as identified and appropriate
  • Facilitation of monthly team meetings
  • Budget/financial oversight of the program
  • Provides program outcomes as appropriate to Jefferson Center mission and values
  • Assist in 27-65 emergency procedures in facilities as needed
  • Facilitate transitions of consumers into and out of ACFs, NH and placements
  • Knowledge of PASRR processes and facilitation of its linkage with mental health service delivery
  • Participate in on-call pool for Jefferson Center Karenโ€™s house and Inn Between
Other Duties:
  • Quality Control/Improvement for case management services/documentation
  • Participation in scheduled Intensive and Center wide training and mandatory in-services
  • Participate in assigned work groups/committees including team meetings, JCMH Management Team meeting, Intensive Network Management Team meeting, external work groups and team meetings
  • Participate in supervision.
  • Participation in BHA site review
  • Program development/implementation to expand Evidence Based Practices
  • Develop/maintain working relationship with BHA and HCPF as well as other community partners.
  • Maintain effective interpersonal relations with consumers, peers, subordinates, upper management, visitors and the general public.
  • Work cooperatively with other community agencies, as appropriate, and in agreement with supervisor.
  • Knowledge/experience in trauma informed care and practices or willingness to obtain training/education on this

Salary Range:ย $67,300 to $85,700*

Additional Salary Information*:

  • The salary range above is based on 1.0 FTE (full time equivalent) or 40 hours per week. Less than 40 hours/week will be prorated and adjusted to the appropriate FTE.*
  • Jefferson Center pay is determined by various factors including education level, licensure level, years of relative experience, and internal equity amongst current staff.*
