Work Environment

This position is primarily remote, allowing for flexibility and comfort while engaging with clients and healthcare partners. Key Responsibilities

• Act as a crucial liaison between clients, their families, and outpatient providers to ensure continuity and quality care.
• Engage with insurance companies for pre-certification and utilization management, streamlining the process for client support services.
• Participate in comprehensive discharge planning and aftercare strategies, ensuring clients have the tools and resources they need post-treatment.
• Provide therapeutic meal support, fostering healthy relationships with food and nutrition among clients.
• Deliver safe, supportive care that is empathetic and affirming of each client's unique identity, facilitating personal healing and growth.

Patient Population / Client Focus

Our primary focus lies in serving older adults, particularly those within hospice care who may be facing complex emotional and psychological challenges. As a Licensed Clinical Social Worker, you will engage with clients in sensitive situations, providing them with necessary emotional support during their transition and ensuring that their dignity and preferences are prioritized in the care they receive.

Qualifications

• A Master's degree in Social Work from an accredited institution is required.
• Must possess a valid LCSW license in the state of Colorado.
• Demonstrated experience working with older adults and those in hospice care settings.

Compensation and Benefits
