Key Responsibilities
  • Act as a crucial liaison between clients, their families, and outpatient providers to ensure continuity and quality care.
  • Engage with insurance companies for pre-certification and utilization management, streamlining the process for client support services.
  • Participate in comprehensive discharge planning and aftercare strategies, ensuring clients have the tools and resources they need post-treatment.
  • Provide therapeutic meal support, fostering healthy relationships with food and nutrition among clients.
  • Deliver safe, supportive care that is empathetic and affirming of each client's unique identity, facilitating personal healing and growth.
Patient Population / Client Focus

Our primary focus lies in serving older adults, particularly those within hospice care who may be facing complex emotional and psychological challenges. Qualifications
  • A Master's degree in Social Work from an accredited institution is required.
