About the Role:

The Home Health RN Clinical Manager plays a pivotal role in overseeing and enhancing the delivery of nursing care within a home health care setting. This position is responsible for managing clinical staff, ensuring compliance with healthcare regulations, and maintaining high standards of patient care. The manager will coordinate care plans, facilitate communication between interdisciplinary teams, and support continuous quality improvement initiatives. By leading clinical operations, the role ensures that patients receive safe, effective, and compassionate care in their homes. Ultimately, the Clinical Manager drives clinical excellence and operational efficiency to improve patient outcomes and satisfaction.


As a Home Health Clinical Manager, you will:

  • Direct and coordinate clinical departments; assume responsibility for continuity, quality, and safety of services delivered in compliance with state and federal regulations. (Conditions of Participation).

  • Promote the philosophy and administrative policies of the Agency, in an effort to ensure quality of patient care.

  • Participate in activities relevant to professional services including development of qualifications and assignment of agency personnel.

  • Supervise and provide direction to subordinates, in an effort to ensure quality, compliance with the Plan of Care, assessment and reassessment of patients' needs, and continuity of services by appropriate health care personnel.

  • Monitor the QAPI Program and assure appropriate corrective measures are performed.

  • Promote compliance with all fiscal intermediary and/or other third party payors, through education, coaching, and other assistance as necessary.

  • Participate as a member of the Interdisciplinary Group (IDG), assists in development of the IDG plan, and participate in QAPI activities.

  • Monitor systems and identifies problem areas to the Administrator both verbally and through written reports.

  • Meet mandatory continuing education requirements of the agency and licensing board.

  • Promote and educate regarding the concepts of infection control and standard precautions in coordinating and performing patient care activities to prevent contamination and transmission of disease.

  • Use effective interpersonal relations and communication skills; facilitate the use of these skills by other team members to achieve desirable outcomes.


Requirements

  • Current RN License

  • Valid Driver’s License

  • Valid Auto Insurance

  • CPR Certification

  • At least 2 years of Home Health management experience

  • HomeCare HomeBase experience preferred
