We are growing!
PRN Home Health and Therapy is seeking an experienced Clinical Manager / Assistant Director of Nursing (ADON) to join our leadership team. 2 years of Home Health required - 1 year of supervisor role preferred. This role is both aclinical leadership and management position, supporting day-to-day clinical operations while ensuring exceptional patient care and regulatory compliance.

The ADON / Clinical Manager will work closely with the Director of Nursing to lead and support nursing staff, oversee clinical services, promote best practices, and contribute to the continued development and expansion of our nursing programs.


Key Responsibilities

  • Coordinate and oversee all direct and indirect patient care services provided by clinical personnel
  • Provide clinical leadership and management support to nursing staff, including guidance with patient and caregiver assessments, care planning, implementation, and evaluation
  • Ensure patient needs are continually assessed and met, including referral management and coordination of care
  • Collaborate with the QA Nurse to review OASIS documentation for accuracy, quality, and regulatory compliance; provide education and coaching to staff as needed
  • Participate in the development, implementation, and evaluation of organizational goals, policies, and procedures
  • Assist with hiring, onboarding, orientation, and ongoing education of nursing staff
  • Support and participate in annual performance reviews and ongoing staff development
  • Help maintain appropriate staffing levels and assist with scheduling to support operational needs
  • Develop, implement, and expand nursing programs and clinical services
  • Serve as a Clinical Manager by promoting clinical excellence, accountability, and evidence-based practice
  • Participate in on-call rotation, including weekdays, weekends, and holidays as required
  • Lead and participate in staff meetings, interdisciplinary meetings, and clinical trainings
  • Travel monthly and as needed to Madison, WI for in-person onboarding, training, and staff meetings

What Were Looking For

  • Active Registered Nurse (RN) license in the state of Wisconsin
  • Degree in Nursing from an accredited program (National League for Nursing preferred)
  • Minimum of 2 years of home health nursing experience with 1 year leadership
  • Previous leadership, supervisory, or clinical management experience strongly preferred
  • Demonstrated ability to lead, mentor, and support a clinical team
  • Strong organizational, problem-solving, and multitasking skills in a fast-paced environment
  • Excellent verbal and written communication skills with strong attention to detail
  • Valid drivers license and active auto insurance
  • Comfortable with regular driving and regional travel
