Job description

Vertus HealthCare is seeking a friendly, empathetic, and detail-oriented Clinical Intake Coordinator/Scheduler to join our growing home health team. As a leading home care agency, we are committed to delivering a higher standard of care across multiple counties.

This role is essential to ensuring smooth coordination of patient referrals and transitions into home health services. The ideal candidate has strong clinical knowledge, excellent communication skills, and a solid understanding of home health regulatory requirements.

Position Summary:

The Clinical Intake Coordinator/Scheduler is responsible for managing incoming referrals, verifying eligibility, and coordinating admissions under the supervision of the Director of Nursing/Clinical Manager. This role serves as a key liaison between referral sources, clinical staff, and administrative teams.

Intake & Coordination:

  • Receive and process referrals from physicians, facilities, and internal staff

  • Enter referral data accurately into EMR systems

  • Verify and ensure accuracy of patient demographic and insurance information (Medicare and non-Medicare)

  • Coordinate timely admissions and transitions into home health services

  • Maintain detailed coordination notes and documentation

Communication:

  • Serve as a primary point of contact for referral sources

  • Communicate referral status, including non-admissions and reasons, to referral sources and Sales Managers

  • Collaborate with clinical and administrative teams to ensure seamless care coordination

Administrative Duties:

  • Monitor and follow up on automated eligibility alerts

  • Coordinate with Billing Manager for insurance verification (non-Medicare cases)

  • Communicate benefits and coverage details as needed

  • Scan and upload referral documentation promptly for field staff access

  • Clearly communicate acceptance or denial of referrals

Additional Duties (as needed):

  • Participate in on-call rotation

  • Perform patient visits if applicable

  • Function as PRN staff nurse when required

Requirements:

  • Graduate of an approved school of nursing (LPN/LVN or RN)

  • Active Florida nursing license

  • Minimum 2 years of healthcare experience

  • At least 1 year of home health experience

  • Strong knowledge of home health regulations, Medicare guidelines, and admissions criteria

  • Experience with EMR systems (Alora Plus preferred)

  • Excellent organizational, communication, and multitasking skills
