What will you do as a PMC Health Coach (LCSW/LCMHC):

A Licensed Clinical Social Worker/Licensed Clinical Mental Health Counselor in a primary care setting is a unique and respected role. The LCSW/LCMHC provides individual emotional health counseling to both established and non established PMC patients. The LCSW/LCMHC can bring their style of counseling into the primary care setting to assist patients with their mental health. The LCSW/LCMHC is a part of a greater wellness team, engaging with those team members as well as primary care provider teams.

A day in the life of a PMC Health Coach (LCSW/LCMHC) may include:

• 95% or more will be attributed to providing emotional health counseling for conditions such as anxiety, depression, grief and loss, caregiver stress, and adjustment disorders

• Recognize patient’s rights in accordance with the Code of Ethics.

• Collaborates with other disciplines; promotes and supports research activities; and utilizes available resources to provide and improve patient care.

• Maintains open lines of communication with providers needed for collaborative care.

• Communicates with clinical staff to ensure patient follow up and continuity of care.

• Assists in marketing of wellness services both internally and externally.

• Assists in development of patient education materials and programs.

• Identifies resources available to patient population.

• Participates in quality improvement team efforts.

• Participates in monthly team meetings.

• Completes appointment scheduling as needed.

• Performs other duties as assigned.

• If knowledge and skill base allows…

o Perform appointments related to emotional barriers to weight loss (Intensive Behavioral Therapy for Obesity)

o Perform Tobacco Cessation Counseling

o Assist with Diabetes Self-Management group classes – emotional eating, relationship with food


Required Qualifications

Education (Including Licenses and Certifications): Current Licensed Clinical Social Worker license required or current License for Licensed Clinical Mental Health Counselor required.

Preferred Qualifications

One or more years of experience with emotional health counseling preferred. Additional experience with clinical health education; weight loss management, tobacco cessation, stress management, or physical activity preferred.


Shift: Day Shift (Monday through Friday) no weekends or holidays

Pay Type: Hourly(Non-Exempt)
