Job Description Summary
The Clinical Assessment Manager is responsible for coordinating with the clinical team for OASIS review and plan of care development. The Clinical Assessment Manager ensures that care and services are delivered appropriately.
Essential Job Functions Responsibilities:
  • Reviews available patient information related to case, including disciplines required.
  • Reviews all OASIS for accurate documentation in conjunction with the OASIS guidelines.
  • Educates staff on OASIS documentation with proper OASIS guidance per CMS.
  • Performs start of care case conference with clinical team to discuss appropriateness of disciplines ordered, specialty pathways, and plan of care development.
  • Reviews and evaluates each case by reviewing the services provided by clinicians, conferences, record review, discusses and verifies impressions, instructs, and guides clinicians to promote more effective performance and delivery of quality home care services, and is always available during operating hours to assist clinicians as appropriate.
  • Reviews patient's clinical diagnosis, medications, procedures, and clinical course.
  • Assist clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing a plan of care.
  • Attend case conference meetings with organization personnel to facilitate coordination of care.
  • Conducts quarterly record reviews and communicates findings and recommendations to Executive Director and appropriate organization personnel.
  • Assists Executive Director in the planning, implementation, and evaluation of in-service and continuing education programs.
  • Assists in the formulation, revision, implementation, and evaluation of policies and procedures, as well as strategic goals and objectives.
  • Complies with accepted professional standards and principles.
  • Participates in public relations and community activities that promote the Organization's role as an effective member of the health care delivery system.
  • Promotes customer service orientation for all organization personnel.
  • Must adhere to the company attendance and reliability guidelines, which are an essential function of the job to ensure quality and consistent patient care.
  • Performs other duties and activities assigned.

Job Qualifications:
  • Registered nurse with current licensure to practice professional nursing in the state of California.
  • Must have OASIS certification prior to starting position.
  • Home Care Home Base (HCHB) experience preferred
  • Graduate of National League for Nursing accredited school of nursing. BSN degree is preferred.
  • Minimum two years of experience conducting quality OASIS reviews in a home health setting.
  • Demonstrate experience in star measures and value-based purchasing.
  • Comply with accepted professional standards and practice.
  • Demonstrates excellent observation, verbal, and written communication skills.

Physical Requirements:
  • The ability to sit frequently for 5-8 hours.
  • The ability to frequently stand for 1-2hours.
  • The ability to intermittently bend or climb for less than 1 hour.
  • The ability to occasionally twist, kneel, crouch and crawl for 1-3 hours.
  • The ability to lift 50 pounds at times.
