Position Summary

1st Choice Healthcare is seeking a detail-oriented and organized Referral Coordinator to support patient care at our Corning clinic.

The Referral Coordinator is an integral member of the care team and is responsible for coordinating patient referrals to specialists, diagnostic facilities, and other healthcare providers. This role ensures accurate patient demographic and insurance information, obtains required pre-authorizations, and tracks referrals to support timely appointments and continuity of care. The Referral Coordinator must demonstrate excellent organizational, written, and verbal communication skills and maintain strict confidentiality while working with sensitive patient information. A professional appearance and demeanor are expected when interacting with patients, families, visitors, and clinic staff.

Key Responsibilities

  • Coordinate and manage patient referrals to specialists and external healthcare providers
  • Ensure complete and accurate patient demographic and insurance information
  • Obtain required pre-authorizations and referrals from insurance carriers
  • Track, follow up, and document referral status to ensure timely completion of services
  • Communicate effectively with patients, providers, insurance companies, and external offices
  • Maintain accurate documentation within the electronic health record (EHR) system
  • Adhere to HIPAA, organizational policies, and regulatory requirements

Qualifications

  • High school diploma or GED required
  • Previous experience in a medical office or healthcare setting preferred; referral coordination experience strongly preferred
  • Proficiency with electronic health records (EHR) and healthcare software systems
  • Strong organizational skills with high attention to detail
  • Excellent written and verbal communication skills
  • Ability to multitask and work efficiently in a fast-paced environment
  • Ability to lift and carry up to 25 pounds

Additional Requirements

  • This is a Safety Sensitive position; use of a Medical Marijuana card is prohibited in accordance with Act 593
  • Facial piercings are not permitted
