PRN Home Health Physical Therapist (PT)

Are you a compassionate Physical Therapist looking for the flexibility of PRN while providing exceptional one-on-one patient care? At Choice Health At Home, you'll have the autonomy to manage your schedule, build meaningful relationships with patients, and help them regain independence in the comfort of their own homes.
What You'll Do
As a Home Health Physical Therapist, you will:
  • Perform comprehensive patient evaluations and establish individualized treatment plans.
  • Develop and implement therapeutic interventions to improve mobility, strength, balance, and functional independence.
  • Evaluate patient progress and modify treatment plans to achieve optimal outcomes.
  • Educate patients and caregivers on exercises, safety, and disease management.
  • Maintain accurate, timely documentation in accordance with company standards.
  • Collaborate with physicians, nurses, and the interdisciplinary care team.
  • Follow infection control and universal precaution guidelines.
  • Deliver compassionate, high-quality care while promoting Choice Health At Home's mission and values.
Qualifications
  • Graduate of an accredited Physical Therapy program.
  • Current Physical Therapist (PT) license.
  • Minimum of one (1) year of home health experience preferred.
  • Current CPR certification.
  • Valid driver's license and reliable transportation with current auto insurance.
  • HomeCare HomeBase experience preferred.
Physical Requirements
  • Ability to stand, walk, bend, kneel, squat, and sit for extended periods.
  • Ability to safely lift, carry, push, or pull up to 100 pounds with appropriate assistance when necessary.
