Job description

First Choice Health Centers is seeking a Nurse Practitioner or Physician Assistant for our family care practice in Manchester, CT. This position is accountable for independently performing a full range of tasks in delivery of direct patient care in an outpatient setting.

Experience and Training:

  • Proven experience as a Nurse Practitioner or Physician Assistant evaluating patients of all ages and establishing care plans
  • A current license to practice as a NP or PA in the state of Connecticut.
  • Appropriate Board Certification
  • A commitment to serving diverse and vulnerable populations with compassion and respect.
  • Excellent communication, interpersonal, and organizational skills.
  • Connecticut Controlled Substance license and DEA license
  • BLS Certification through the American Heart Association or American Red Cross.
  • Pay is based on several factors including but not limited to work experience certifications, etc. Range for this position is $115,000 - $160,000

Standard Job Duties:

  • Evaluates, assesses patient illness/injuries assessing all ages.
  • Chronic care management.
  • Performs physical examinations, and special examinations.
  • Takes medical histories.
  • Develops plans of care.
  • Provides routine and illness care.
  • Orders medications; orders diagnostic tests; and evaluates results.
  • Assumes responsibility for patient follow-up, coordination, and referrals to other health care providers as necessary.
  • Maintains patient medical records.
  • Provides patient counseling/education.
  • Participates in medical student and health professional teaching.
  • Performs related duties as required.
